– Click the field and select the value for cornea size.

– Click the field and select the value for pupil size.

field.

The Keratometry panel displays current encounter exam findings, as well as findings for the last five encounters, in separate grids for OD and OS on the Prelim Exam - OPH and Provider - OPH templates. keratometry panel data The Keratometry panel can be found on multiple templates, including. Document a Lensometry Exam
Expand the Lensometry/Refraction panel if not already expanded. Select the Glasses Rx Verified By check box to indicate that you verified the glasses prescription. The current user name fills the corresponding field. In the Lensometry section, enter OD values in the following fields: SPH CYL Axis - enter a numeric value ADD Prism 1 Base 1 Prism 2 Base 2 Repeat step three to enter OS values. Document a Contact Lens Fitting and Evaluation Visit
Depending on the practice, workflow on the Contact Lens Evaluation - OPH template may begin after intake or after preliminary exams performed by a technician. Any information previously documented during the encounter appears in the corresponding data fields on the Contact Lens Evaluation - OPH template. This information can be reviewed and updated as appropriate. In the Reason for Visit panel, the provider reviews the technician's documentation, makes updates, and signs-off on the HPI information. In the Visual Acuity panel, enter exam findings or review and update previously documented results. visual acuity panel In the General Information panel, enter or review and update the patient information related to wearing contact lenses. general information panel In the Contact Lens History section, review and select the current contact lens prescription from the grids. contact lens history panel The prescription values appear in the fields of the Current Contact Lens Prescription section.
