Add, Edit, and Delete Custom Words in the SOAP Note

To ensure accurate dictation results, you can add custom words to your speech profile and save them in categories, such as person names, location names, medicines, procedures, and diagnoses. For example, you can replace similar-sounding provider names with different spellings, such as Carter and Karter. You can also add abbreviations, such as CHF, to appear in the transcription when you dictate congestive heart failure. Open NextGen® Mobile. Open the Patient Chart view. Select Today's Visit. Note: You must add or select an unlocked encounter. Select Dictate. In SOAP Note, select the SOAP section, and then select the microphone icon to dictate a note. To stop or pause the dictation, select the microphone icon. Notes are entered in the SOAP section, and Tap to start changes to Tap to continue. Double-click the misrecognized word, and then select Dictionary from the menu. Add New Word opens and Word to Replace displays the selected-misrecognized word. In Replacement Word, enter the replac

Copy Forward SOAP Note Data from Previous Encounters

You can copy previous Subjective, Objective, Assessment, and Plan (SOAP) notes, and then send the notes to NextGen® Enterprise EHR. Open NextGen® Mobile. Open the Patient Chart view. Select Today's Visit. Note: You must add or select an unlocked encounter. Select Dictate.The SOAP Note view opens. Select a SOAP section to enter clinical documentation. Select Copy Forward.The previously entered data for that SOAP section appears with the encounter dates. Recorded soap notes of previous encounters To copy a note to the today's visit SOAP note text box, select the note. Note: You can copy notes from multiple encounters. Current encounter soap note text box Optional: Edit the copied notes in the SOAP note text box using the mobile keyboard, and then select Done on the keyboard. Edit SOAP Settings

Open NextGen® Mobile. Open the Patient Chart view. Select Today's Visit. Note: You must add or select an unlocked encounter. Select Dictate.SOAP Note opens. Select SOAP Settings, and select the appropriate options. To create macros, do the following: Select Create New Macro. Enter a macro name and macro text. Optional: To turn on NextGen® Ambient Assist macros, select Ambient, and select a mode. Standard adds defined words without additional words outside of dictation. Ambient Complete adds additional words between existing macro text based on context. Note: The added macros are also available in the NextGen® Mobile Cloud Portal. Enter the activation phrase to add the macro. Optional: To map a SOAP section to a macro, select SOAP Section. Note: You can select only one SOAP section for a macro. Select Done. Documenting SOAP Clinical Notes

In Today's Visit in Patient Chart, you can select Dictate to document and complete clinical notes. In Subjective, Objective, Assessment, and Plan (SOAP) Notes, you can dictate your notes using a microphone, add blocks of text with documentation accelerators like Copy Forward or Macros, or enter and edit notes using a keyboard. On completing the SOAP notes, you can send the notes to NextGen® Enterprise EHR. Today's visit view You can use SOAP Settings to modify your SOAP sections in SOAP Note. Copy forward button Note: If the device is offline while searching for the past encounter dictation text, a message appears indicating that the device is offline, connect to the internet and try again. The Histories and Previous Diagnostics status depends on the following NextGen® Enterprise EHR versions: NextGen® Enterprise EHR Version Status in NextGen® Mobile 5.9.3 and later Not available 5.9.4 to 6.0.0 Turned off 6.1.0 Turned on NextGen® Mobile integrates with Google™ Speech Recognition Engine

Send SOAP Notes

You can complete Subjective, Objective, Assessment, and Plan (SOAP) notes and then send the notes to NextGen® Enterprise EHR. Note: NextGen® Speech-to-Text (STT) is available only with an established internet connection and is not currently available for offline use. The transcription system can process and refine only content greater than fifty characters in length. Open NextGen® Mobile. Open the Patient Chart view. Select Today's Visit. Note: You must add or select an unlocked encounter. Select Dictate.SOAP Note opens. Note: If necessary, you can edit the SOAP settings. To dictate a note, select the SOAP section, and then select the speech icon. Alternatively, you can use the keyboard to enter or edit dictation notes in the SOAP section, and then select Done on the keyboard. Note: You can edit the dictated note using both keyboard and verbal commands when the transcription system processes and completes the transcription of the dictated note.
