Patient Allergy Trigger Overview

You can run the Patient Allergy trigger to add an allergy directly to the patient's allergy list instead of using the Medication Allergies module. The trigger action updates the module. The Patient Allergy trigger is set up in Template Editor. Note: If you execute this trigger to add an allergy that already exists, a message appears, asking if you want to add the allergy again. You can select Yes to add the allergy again, or select No to cancel the action. The trigger may be set up to add multiple reactions for an allergy, up to a maximum of five. You may also be able to manually enter a reaction in a data entry field. The system locates and stores any SNOMED codes associated with allergy reactions. If a Drug Utilization Review (DUR) is set up, you can check for interactions between each patient allergy that is added and all of the active medications prescribed to the patient. Problem List

The Problem List tab enables you to view the details of the active, inactive, resolved, and unresolved problems of a patient and update them as needed. You can record and update a patient’s problems using the Systemized Nomenclature of Medicine - Clinical Terms (SNOMED-CT®) concept. SNOMED-CT® is a standard vocabulary used to express clinical terms. SNOMED-CT® is comprised of descriptive terms or synonyms related to a clinical concept and provides links between different concepts. The problem list is demographic and is not associated with an encounter. After you record a problem if you need to use it as a diagnosis for billing, you must add the problem to the billing ICD list. Add a New Problem to a Patient's Problem List

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Care Transitions tab, expand the Problem List after Discharge panel, and then click Add. The Problems window opens. Click Add Problem. A search window appears. Add Problem Window Note: Problems are stored in the database and displayed using SNOMED® CT (SNOMED Clinical Terms). Search for the problem by typing a full or partial name of the problem in the search box. Note: The Add to My Tracked Problems option is selected as a default. Click the desired problem. Click Select to display and add any additional details. Make any necessary changes to the problem. Click Accept to add the problem to the list. Select an Assessment from Clinical Problems

On the Add or Update Assessment template, Clinical Problems section, filter the grid by selecting Show chronic, Show my tracked problems, or No active problems. Assessments Select a Clinical Problem item from the grid. A system check determines if there was a previously mapped ICD code for that clinical problem for the open patient chart. If there is a previously saved mapping, that ICD code is added to the ICD description and ICD Code fields to be used for today’s assessment. The Clinical Problems section includes the Systematized Nomenclature of Medicine - Clinical Terms (SNOMED - CT®) problem list (from the Concept Id column on the Problem List tab in the Problems module). The system translates the selected diagnosis into a SNOMED® code and saves it in the Clinical Problems section.
