Clinical Problem DUR Preferences

This section outlines the preferences that are used to perform clinical problem interactions DUR: In System Administrator: Setting up the Medications Operations Right for Interaction Display Level. You can select User Group in Enterprise Practice > Rights > OPERATIONS > Medications. Acknowledge Allergy - Drug Interactions

The DUR checks the selected allergy against the list of all active medications. If any interactions are found, they appear on the Interaction Check DUR window. Note: User-defined allergies and medications are not checked for interactions. The display of allergy interactions is based on the display levels set for the system and/or a specific user. If the Show DUR across enterprise option is enabled, NextGen® Enterprise EHR checks for possible interactions with the patient's medications across all practices. CAUTION: You must review and acknowledge each interaction alert. If you select the Cancel button, your action is audited, and you must enter the reason for cancellation. Take the necessary steps to avoid an adverse reaction for the patient. Repeat the following steps for each interaction alert. Select the interaction alert. Check Medication Allergy Interactions

You can check for interactions between a patient allergy and all of the active medications prescribed to the patient. Open the Medication Allergies module. Select an active medication allergy. Select Interactions. NextGen® Enterprise EHR checks the selected allergy against the list of all active medications. If there is an interaction, the Interaction Check DUR window opens. Interaction Check DUR Note: The display of allergy interaction is based on the display levels set for the system, a specific user, or both. Highlight the listed medication to view the summary of the existing conflict. NextGen® Enterprise lists a summary of effect and level of risk. Select OK to close the Interaction Check window. Medication Interactions

A new medication prescribed to a patient or an existing medication being renewed can have a negative interaction with either an allergy or another medication previously prescribed. The medication can create a disease contraindication or be a duplicate of an already prescribed medication. The medication interaction process checks for all possible instances and displays a Drug Interaction window that lists all interactions, contraindications and duplications. The Drug Interaction window display of drug and allergy/drug interactions, disease contraindication, and geriatric and pediatric precautions is based on the display levels set for the system and a specific user. The medication interaction process does not perform checks on user-defined medications and allergies. Resolve Medication Contraindications or Drug Interactions

If a medication item that you selected for import has contraindications or interactions, the Drug Interaction window appears. Drug Interaction Window Either confirm the contraindication/drug interaction message or choose to send an alert containing drug interaction information to another provider in NextGen® Enterprise EHR. Note: The application does not perform a drug interaction check for allergies, problems, immunizations or diagnosis codes. You can manually check for interactions in the pertinent NextGen® Enterprise EHR module after all reconciliation tasks are completed. To support the sending of alerts to providers, an NextGen® Enterprise EHR Workflow IREM Import agent must have previously been created and configured. Drug to Allergy Interaction

A new medication prescribed to a patient can have a negative interaction with a patient's medication allergy. If such interaction occurs the Drug Interaction window opens. The Drug Interaction window lists the medication that caused the interaction, and information for each listed patient medication allergy. The Reference at the bottom of the window displays the citation for the DUR information. You must acknowledge all notices listed in the Drug Interaction window and specify the reason for overriding the warning. The medication interaction process displays drug interactions according to the display levels set for the system and a specific user. The medication interaction process does not perform checks on user-defined medications or user-define allergies. If the Enterprise option to check for interactions across practices is enabled, the medication interaction process checks for possible interactions across all practices.
