Open the Problem Notes
Add notes to a problem in Problems module in the
Problem Notes
window.
The
View/Add Notes
button enables you to view or add notes to a problem in the Problems module.
Select the patient's encounter that contains the problem.
Select the problem in the
Problem List
.
Do one of the following:
Right-click the problem, and then select
View/Add Notes
.
On the problem list data entry area toolbar, select
View/Add Notes
.
The
Problem Notes
window opens.
Set Column Preferences for Problem Notes Window
Set the display preferences of columns in the
Problem Notes
window.
Add a Problem Note
Add relevant information about a problem as a note in the patient chart.
Update a Problem Note
Edit the problem note or add more information in the
Problem Notes
window.
Delete a Problem Note
Delete problems notes that are incorrect or no longer relevant.

Resolve an Active Problem
You can set a selected active problem to the resolved state. Select the patient's encounter that contains the problem to resolve. Select an active problem in the Problem List. Do one of the following: Right-click the problem, and then select Resolve. On the problem list data entry area toolbar, select Resolve. Note: If you select a resolved problem in the display area, the Resolve option is not available. The Problem Status field updates to display Resolved. The Resolved Date field is selected and displays the current date. If necessary, change the Resolved Date. When you change the Resolved Date, the Resolved By field displays your user ID and the Resolved Reason field becomes available. Enter the appropriate Resolved Reason. Mark a Problem as Chronic
You can set a long-standing or recurring problem displayed in the problem list as chronic. Select the patient's encounter that contains the problem. Select a problem in the problem list that is not marked as Chronic. Do one of the following: Right-click the problem, and then select Set Chronic. On the problem list data entry area toolbar, select Set Chronic. Note: If you select a problem that is already marked as Chronic, the Set Chronic option is not available. Add a Problem Note
You can add or update a problem note to a patient's chart when required. The problems toolbar contains a Recent Note column that displays the content of the most recent note added for the problem. Select the patient's encounter that contains the problem. Select a problem in the Problem List. Do one of the following: Right-click the problem, and then select View/Add Notes. On the toolbar of the Problem data entry area, select View/Add Notes. The Problem Notes window opens. Problem Notes Select Add Note. The Add Problem Note window opens. Update a Problem Note
Select the patient's encounter that contains the problem. Select a problem in the Problem List. Do one of the following: Right-click the problem, and then select View/Add Notes. On the toolbar of the Problem data entry area, select View/Add Notes. The Problem Notes window displays the existing notes. Problem Notes Select the note in the Problem Notes window, and then select Update Note. The Edit Problem Note window displays the current note. Update the note as required. Problem List Data Entry Area Toolbar
The problem list data entry area toolbar is located at the top of the data entry area and below the data display area. The data entry area toolbar buttons enable you to perform functions related to a patient's problems such as add, recode, and modify the problems. Set Column Preferences for Problem Notes Window
To view the list of settings for the columns, select the Preferences button in the Problem Notes window. Select or clear the applicable columns to view or hide them in the Problem Notes window. Set Columns to Display To specify which columns to view in the Problems Notes window and their display order: Select Set Columns to Display. The View Preferences window opens. To add a column, select the column name in the Available Columns list, and then select Add. To add all columns, select Add All. To remove a column, select the column name in the Display these fields in this order list, and then select Remove. To remove all columns, select Remove All. To set the display order of a column, select the column name, and then select Move Up or Move Down.
