Family Information

In the Family Information section of the UDS tab, you can enter family size and annual income for the patient's head of household. This information is typically used by Community Health Centers in conjunction with sliding fee schedules. When a patient qualifies for a sliding fee schedule, family size and income are used to determine the percentage of charges that are automatically adjusted for the patient. Family information is also reported on the UDS Report. Note: A head of household must be selected for a patient to enter family information. Family Information is stored at the practice level; therefore, each practice can enter and maintain different information for a person or patient. Relationship Information

In the Relationship section of the Relations/Roles tab, you can enter other people that are associated with a person or patient in the practice. For example, a patient may have relationships to a guarantor, an insurance policyholder, an emergency contact, and a head of household. Relationship information is stored at the system level; therefore, all practices in all enterprises share the same information for a person or patient. Head of Household Information

In the Head of Household section of the Relations/Roles and UDS tabs, you can enter the person designated as the head of household for a patient. This may be the patient, or another person such as a parent or spouse. Family information, which includes family size and annual income, can then be entered and associated with the head of household. The information is saved for established patients with an existing chart, and for new patients when the chart is created. Note: A head of household must be selected for a patient to enter family information. Head of household information is stored at the practice level; therefore, each practice can enter and maintain different information for a patient. Change a Head of Household - Relations/Roles Tab

Open the Patient Information window for the desired patient. Select the Relations/Roles tab. The Head of Household section displays the current head of household. Delete the current head of household (see Delete a Head of Household - Relations/Roles Tab). Add a head of household (see Add a Head of Household - Relations/Roles Tab). Delete a Head of Household - UDS Tab

Open the Patient Information window for the desired patient. Select the UDS tab. The Head of Household section displays the current head of household. Select the Open Record button, and select Delete. The head of household is removed. Note: The deleted head of household is not removed from the Relationship section of the tab. Change a Head of Household - UDS Tab

Open the Patient Information window for the desired patient. Select the UDS tab. The Head of Household section displays the current head of household. Select the Open Record button, and select one of the following: To change the head of household to the patient, select Self. The patient displays in the Head of Household section of the UDS tab. To change the head of household to someone other than the patient, select New. The Head of Household Lookup window appears. Enter the desired Search Criteria to find the head of household. Select Find. All persons and patients that match the search criteria entered display in the Matching Records list. If no records are found, the following prompt appears: No records were found! Do one of the following: From the Matching Records list, double-click to select the head of household. Select New to add a person record for the head of household.
