Generate Diabetes Report of Patients Filtered by Gender and Race

You need to generate a report that lists Asian-American female patients over 40 who have been diagnosed with diabetes during the last year. From the Report Filter, you would filter the data by accessing the following modules and options: Select Diagnosis and then select the specific diagnosis (for example, Diabetes). Filter the report data. Filter the patient demographics (for example, gender, race), using the Patient option. After making all of your filtering and data selection choices, generate the report. Generate Diabetes Report of Patients at Risk for a Diagnosis

This report needs to identify patients who have received lab results of a glucose level greater than 126 two or more times, and who have never been diagnosed with diabetes. To create this report, you would filter the data by accessing the following modules or options from the Report Filter Settings List: Orders (for example, Order Type: Laboratory; Test Results: Glucose, two or more times greater than 126) Diagnosis (for example, Diabetes) Patient (Patient demographics filter) After making all of your filtering and data selection choices, generate the report. Generate Asthmatic Report of Patients Who Smoke or are Exposed to Second Hand Smoke

This report needs to identify patients with a specific diagnosis who have a documented concern or risk. In this case, the report should display patients who are diagnosed with Asthma and either smoke or are exposed to second hand smoke. To create this report, you would filter the data by accessing the following modules or options from the Report Filter Settings List: Templates (for example, Social_History, Tobacco_usage) Provider (for example, All) Diagnosis (for example, Asthma) Enterprise (for example, All practices) Filter (for example, selected Social History template fields) After making all of your filtering and data selection choices, generate the report. The Report Filter enables you to create simple or complex reports. From the Report Filter, you can: Select a module or option from the Settings List to filter module and patient data. Select a combination of modules or options from the Report Filter Settings List to filter module and patient data. Select additional informational and display options, including sorting, columns, and customizing the header or footer, prior to saving and/or generating a report. Select Stored Procedure Data (Enterprise-level NextGen® Enterprise EHR Reports Only)

The Stored Procedure Data function is an advanced reporting feature. To use this function, you must have advanced knowledge of the NextGen database structure and SQL knowledge. If you have questions about using this function, contact your System Administrator. Select SP from the Report Filter Settings List. Note: Stored Procedures Reports are available for Enterprise-Level Reports ONLY. Report Filter Select whether you wish to filter on a Stored Procedure or a View. Select the Stored Procedure or the View from the list. Note: The "ngsprep_care_guidelines" is an SP report when run, pulls the record of all patients (in alphabetical order) due for clinical guidelines. Select Template Data

You can select Template Data for a report. Select Templates from the Report Filter Settings List. In the Select Template pane, select the desired template. If necessary, enter the name of a template to search for. Note: The following Care Guideline templates are no longer available for reporting: CARE_GUIDLELINES CLINICAL_GUIDELINE_HISTORY CLINICAL_GUIDELINE_REASONS CLINICAL_GUIDELINE_UPDATE CLINICAL_GUIDELINES To generate a report that lists the record of all patients (in alphabetical order) due for (selected) clinical guidelines, run the “ngsprep_care_guidelines”. In the Select Field pane, select all the necessary template fields. From the Latest Value By list, select whether to run the report by Modify, Encounter Date or None. To convert all measurements into numeric values for the report, select the Convert to Numeric check box.
