Generate Diabetes Report of Patients at Risk for a Diagnosis

This report needs to identify patients who have received lab results of a glucose level greater than 126 two or more times, and who have never been diagnosed with diabetes. To create this report, you would filter the data by accessing the following modules or options from the Report Filter Settings List: Orders (for example, Order Type: Laboratory; Test Results: Glucose, two or more times greater than 126) Diagnosis (for example, Diabetes) Patient (Patient demographics filter) After making all of your filtering and data selection choices, generate the report. Generate Reports of All Patients Due for Selected Clinical Guidelines

This report provides a list of all patients (in alphabetical order) due for (selected) clinical guidelines. This report needs to be run at the Enterprise level. To create this report, you would filter the data by accessing the following modules or options from the Report Filter Settings List: Select Stored Procedure check box and then select “ngsprep_care_guidelines”. Filter the report data. Filter the patient demographics (for example, gender, race), using the Patient option. Filter the Care Guideline fields using the individual columns. Generate Asthmatic Report of Patients Who Smoke or are Exposed to Second Hand Smoke

This report needs to identify patients with a specific diagnosis who have a documented concern or risk. In this case, the report should display patients who are diagnosed with Asthma and either smoke or are exposed to second hand smoke. Generate Pediatric Report of Patients at Medication Risk

This report needs to identify patients on a specific medication type, which may be a risk for that age group. The report should display medications for pediatric patients who are taking specified SSRI's under the age of 18 by Provider and include Totals. To create this report, you would filter the data by accessing the following modules or options from the Report Filter Settings List: Medication (for example, Patients who have ever taken Zoloft, Paxil, Prozac, Celexa, Lexapro) Provider (for example, All) Patient (for example, Date of Birth, which occurs on or after a specified date) Columns (for example, selecting patient data and report columns, such as Brand Name) Sorting (for example, sorting by Rendering Provider and group) Totals (for example, including the Prescribed Total of the specific medications) Note: In this case, Totals is the last Report Filter option to select. Generate a Diagnosis Report

In this example, you need to generate a report for all patients who have been diagnosed with diabetes and who have blood pressure greater than 140/90 in a period of one year. Open the Report Filter. Select Diagnosis from the Settings List. The Diagnosis window opens. Select to display the Diagnosis Search window. Enter the ICD code 250 in the field. Note: The ICD Code option is the default field. Diagnosis Search Select Search. The list of diagnoses appears. Diagnosis Search Select diabetes mellitus diagnosis description. Select OK. The Diagnosis Search window displays the results in the search pane. Report Filter To select the vital signs, select Templates. Enter vital in the Search Template Name field. The templates appear.
