Document Display Settings

The Document Configuration template enables you to generate a document in run time based on preferred settings. You can configure the document elements at the provider and practice levels, but not the document format. The practice template provides the ability to: Set up general, practice-level options and how information displays on documents for: Orthopedic encounters Vital signs Behavioral Health encounters Set up provider-level content for these document types: Chart Note Consult Note Referral Letter When Chart Note is selected as the default document to print from a patient encounter, the master document is set to generate offline. The practice decides the type of documents to be routed to the Provider Approval Queue (PAQ) for sign off. Vital Signs

You can add, edit or remove vital signs for a patient using the Vital Signs panel. In the Vital Signs panel you can view vital signs documented during this encounter. You can edit ot remove previously entered vital signs and enter new vital signs. The Vital Signs panel is also available on other office visit templates. The Historical information entered this encounter link opens the Vital Signs History template filtered to display only vital signs documented with patient's encounter. Using the History link, you can review the patient’s entire vital signs history on the Vital Signs History template. Using the Graphs link, you can view vital signs in graphical format on the age-appropriate VS Charts template. Vital Signs The vital signs templates are based on age. The age-appropriate template opens depending on whether the patient is less than two years old, equal to or greater than two years old, or an adult. Reading the Vital Signs Monitoring Grid

To read the Vital Signs Monitoring grid, take into consideration the following: The system calculates the patient's weight in kilogram and displays in the Wt(kg) field. If you document patient's weight in the Wt(kg) field, the system calculates the patient's weight in pounds and displays in the Wt(lb) field. The height and weight details documented with any set of vitals appear above the grid in the Baseline Vital Signs. Based on the height and weight data, the system automatically calculates the Body Mass Index (BMI) and Body Surface Area (BSA) and displays next to the labels BMI and BSA respectively above the grid. The Height, Weight, BMI, and BSA values auto-update as you enter new values in the height and/or weight fields in the Vital Signs section. The left and the right arrows at the bottom of the Vital Signs template are used to navigate to the previous or the next available vital signs records. Enter Vital Signs

Use the Vital Signs link to document vital signs obtained during the visit. Select the Vital Signs link. The Vital Signs template opens. Select the Add button on the Vital Signs template. The Vital Signs (Adult or Pediatric) template opens. Vital Signs template Document vital signs information such as Height, Weight, BP and Pulse. Select Save. Select Close to close the template. Vital Signs Monitor

The Vital Signs Device section displays the following: The vital signs monitor name that is connected through COM port. The first time when you connect the device to your personal computer and launch the interface, you may need to click the drop-down arrow to select the device name. The system then makes the selected interface as the default when you subsequently launch the interface. The Monitoring Mode field displays Continuous to indicate that the vital signs need to be captured continuously for the specified timeframe. If you connect a different vital signs device, the Monitoring Mode field is available for selecting the monitoring mode. You can click the drop-down arrow to select the mode of monitoring the vital signs of the patient. The Patient Information section displays the following: The patient's information uploads from the patient chart that is open in the NextGen® Enterprise EHR. Record Vital Signs for the Nurse Progress Note

On the Vital Signs panel, click Historical information entered this encounter to view all vitals for the current encounter. Click History to view a complete history of the client's vitals. Click Add to add a new set of vitals. The Vital Signs (Lite) - [New Record] template appears. The Measured date, Time, and Measured by fields are populated. However, the date and time can be manually changed but the user name in the Measured by field cannot be modified. Enter information using the on-screen numeric keypad. Click the NEXT button below the numeric keypad to enter information in the next field. Select the Carried forward option to pull a previously entered Height into this field. After a Height and Weight value are entered, a BMI value is calculated below the numeric keypad. Select the BMI Plan link to open the Health Promotion Plan template, where an out-of-range BMI can be easily addressed with diet, exercise, and referral information.
