You can indicate a patient's risk level using the Risk-Stratification template.
- Open NextGen® Enterprise EHR.
- On a patient encounter, open any of the standard visit templates (for example, the Intake) template.
- On the template header, select the Risk Level link.
The
Risk-Stratification template opens.
Note: You can see the score in the Diagnosis based risk score field only when an HCC enabled diagnosis code is associated with the patient.
- Select the applicable column to assign a risk level to a patient.
NextGen® Enterprise EHR automatically fills the Modified by and Date fields.
- Enter your comments.
- Select the following check boxes if applicable:
- Include on Master document
- Include on Patient Plan document
- Select Save & Close.
The updated level appears to the left of
Risk Level with a count indicating the selected risk level (1-6) and the associated color.
