Add Problem to the Problem List
To open the Problems window, do one of the following: In the Problem List panel, click Add/Update Click Problems in the patient information bar. The Problems window opens. Enter a problem in the diagnosis search window and click Search. Highlight the problem from the list and click Select. The system fills the associated values in the Fully Specified SNOMED® Description, Problem Description, ICD Description and ICD Code fields in the Problem List panel. For all new problems, Mark as Chronic is selected by default. Enter the Clinical Status. Enter the Onset Date. Enter the Resolved Date. If the problem is not chronic, do one of the following: Select Secondary Condition to mark the problem as a secondary condition. Select Marked as Tracked Problem to add the selected problem to the tracked problem list. To add the problem to the assessment list, select Add to Today’s Assessments. Problem List
The Problem List tab enables you to view the details of the active, inactive, resolved, and unresolved problems of a patient and update them as needed. You can record and update a patient’s problems using the Systemized Nomenclature of Medicine - Clinical Terms (SNOMED-CT®) concept. SNOMED-CT® is a standard vocabulary used to express clinical terms. SNOMED-CT® is comprised of descriptive terms or synonyms related to a clinical concept and provides links between different concepts. The problem list is demographic and is not associated with an encounter. After you record a problem if you need to use it as a diagnosis for billing, you must add the problem to the billing ICD list. Add Problems in the Problem List
You can add a problem to the problem list of a patient. Open NextGen® Enterprise EHR. Open the Histories template. In the Problem List panel, select Add/Update. Enter a problem in the diagnosis search window, and select Search. Select the problem from the list of the search result, and select Select. Select the applicable ICD code, and select Select. The application automatically fills the associated values in the Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code fields in the Problem List panel. Note: For all new problems, the Mark as Chronic check box is selected by default. In Clinical Status, select the status of the problem. In Onset Date, enter the date of onset. In Resolved Date, enter the date when the selected problem is resolved. If the problem is not chronic, to mark the problem as a secondary condition, select Secondary Condition. Add a New Problem to a Patient's Problem List
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Care Transitions tab, expand the Problem List after Discharge panel, and then click Add. The Problems window opens. Click Add Problem. A search window appears. Add Problem Window Note: Problems are stored in the database and displayed using SNOMED® CT (SNOMED Clinical Terms). Search for the problem by typing a full or partial name of the problem in the search box. Note: The Add to My Tracked Problems option is selected as a default. Click the desired problem. Click Select to display and add any additional details. Make any necessary changes to the problem. Click Accept to add the problem to the list. Set up ICD SNOMED Map Trigger
The ICD SNOMED action enables users to view and assign the following: ICD code(s) mapped to a selected SNOMED-CT problem to today's assessment or encounter If only one ICD code is associated with the selected SNOMED problem, the ICD description, ICD diagnosis code ID, and the ICD code are automatically assigned to the selected variable or template field. If more than one ICD code is associated with the selected SNOMED problem, the user must select an ICD code from a list in a window to assign the ICD description, ICD diagnosis code ID, and the ICD code to a variable or a template field. SNOMED concept ID(s) mapped to a selected ICD code to the patient's clinical problem list If only one SNOMED problem is associated with the selected ICD code, the SNOMED concept ID, SNOMED description, and the SNOMED fully specified name are automatically assigned to the selected variable or template field. Access Clinical Decision Support from the Problems Module
You can access the Clinical Decision Support resource from the Problems module. The Clinical Decision Support resource collects ICD-9, ICD-10 (as available), or SNOMED CT code information for the selected problem and the patient’s age, age range, and gender. The information is passed through the InfoButton to the external system. Open the Problems module for a patient. Enter a new problem in the Problem List tab or the Billing ICD List tab, if required. Note: The Problem List tab has the SNOMED CT term while the Billing ICD List tab has the ICD-9, or where applicable, ICD-10 code. Do one of the following: Right-click the problem, and then select Resources > Clinical Decision Support. Select the problem, and then select Resources > Clinical Decision Support from the Problems module toolbar. The application directs you to the external URL set up in File Maintenance.
