When connected to a cancer registry, the Patient’s diagnosis code must match a valid ICD9 and ICD10 codes established by the Cancer Registry community to identify an encounter for a patient with cancer. Use the following procedure to review the list of valid codes. Your system administrator must grant you rights to perform some of the following steps. Quality Reporting Compliance

To meet the 2015 Quality Reporting certification objectives, eligible providers must have the capability to identify and report cancer cases to a public health central cancer registry (except where prohibited) that is in accordance with applicable laws and practices. NextGen® Enterprise EHR offers full support for the defined submission of cancer case information from CEHRT to a public health central cancer registry for the entire NextGen® Enterprise EHR reporting period using the HL7 CDA R2 Standard. A CDA is a document markup standard that specifies the structure and semantics of clinical documents for the purpose of exchange. This flexibility in practice workflows enable you to capture the case information from an organized and convenient location in the system. Cancer Registry Reporting

During a patient encounter, the cancer registry functions enable you to document the diagnosis, evaluation, and treatment of active cancer for cancer registry reporting purposes. When the encounter is concluded and locked, the application generates a cancer report that is sent to the national cancer registry through an HL7 interface. By capturing these data elements, providers can meet the 2015 Quality Reporting certification objectives. To satisfy cancer registry reporting guidelines, you must record the following types of information. Patient demographic information In addition to the general patient data, the national cancer registry requires documentation of specific demographic data points, including race, ethnicity, occupation, and nation of birth. General cancer diagnosis and treatment data This includes data such as onset date, cancer notes, radiation notes, and problem description. Patient Documents

Patient documents in NextGen® Enterprise EHR can be a medical or lab report, a referral form, a professional letter, or any other custom-designed form. You can generate new patient documents from previously created user-defined format documents in the Document Builder. When a patient document is generated, macros in format documents insert data from selected template fields. A patient document can be created locally or remotely on a remote workstation. The patient documents are attached to the applicable encounter in the patient's chart. The patient's chart may also contain documents created outside Document Builder. Third-party documents are imported through the NextGen® Interface Engine Document Import Agent. NextGen® Health Information Exchange (HIE) and Continuity-of-Care (CCD) documents are attached to the NextGen® HIE Documents Category of the Categories tab. Generating a CCD

To generate CCDs, your practice must purchase NextGen® Clinical Interface Bundle including all applicable Rosetta agents. After NextGen® Clinical Interface Bundle is installed and configured, you can run the Generate CCD trigger on a NextGen® Enterprise EHR template. Depending on the trigger setup, a CCD can be generated for a selected patient encounter or all patient encounters (longitudinal). You can also choose to generate CCDs as local files or export them to an external system such as a NextGen Health Information Exchange or Health Information Service Provider (HISP). View HIE Document Category Items

Before you begin To display the HIE documents on the Categories tab, you must configure the Sent HIE Document Category option in the StoreHIEDocuments Rosetta agent. In NextGen® Enterprise EHR Categories tab, you can view the imported, and exported Health Information Exchange (HIE) document categories in the My Categories tab based on user-level configuration. Open NextGen® Enterprise EHR, and select a patient from the Patient Lookup window. Select the Categories tab. Select .The My Patient History Settings window opens. Under the My Patient History Settings | Categories section, select Select Categories.A window opens displaying a list of available categories. To display the imported HIE documents, select HIE Documents category from the list. To display the exported HIE documents, select HIE Documents (Sent) category from the list.
