Document General Information

After reviewing the available patient details on the Home Page template, you can document the general information of a patient in the General panel. Open NextGen® Enterprise EHR. Open the Intake - Cardiology template. Click the General panel. Select the patient. If someone else is providing the patient details, select the relationship of the person with the patient in the Historian field. To capture the summary of care record information for a patient, click Transitioning into Care, and fill the information in the resulting window. To show the HCC links, badges, and grid columns on the templates, select Enable HCC for this patient. To indicate the disability, select Disability. Copy Historical Free Form HPI to Current Encounter

You can copy historical Free Form HPI data within the last three years to the current encounter in the Reason for Visit panel of the Intake – Cardiology template. Before you begin You must configure the Copy Historical Free Form HPI settings on the Practice Configuration template to copy historical Free Form HPI details to the current encounter. Open NextGen® Enterprise EHR. Open the Intake - Cardiology template. Click the Reason for Visit panel. In the left reason for visit pane, click Freeform HPI. The Free Form HPI template opens. Click Copy Forward. Note: If HPI content exists on the Free Form HPI template, the system alerts you that existing findings are overwritten if you select an HPI to copy. Freeform HPI The Copy Forward Free Form HPI Histories template opens with the Free Form HPI entries from the last three years in the grid, with the most recent encounters listed first. Document Reasons for Visit

You can document the reasons for the visit of a patient in the Reason for Visit panel. Open NextGen® Enterprise EHR. Open the Intake – Cardiology template. Click the Reason for Visit panel. To add a Reason for Visit, do one of the following. To enter the reason for the visit and do not want to enter the history of present illness, select the Do not launch HPI check box. From the left pane, select a reason for visit. Click the Other list, select a reason for a visit from the list, and then click OK to get the corresponding window to fill in the information. The reason for the visit is added. To enter additional information for the provider to review, click the Intake Comments link, and type your comments. When you enter additional information, the Intake Comments link appears purple on both the Intake and SOAP tabs. For each symptom, do the following. Access the Reason for Visit/HPI Template

The patient who presents for an Initial Prenatal visit is there because of pregnancy. It is not required to document the same in the Reason for Visit panel. The Reason for Visit panel on the Intake template is commonly used for pregnant patients who are visiting for a problem outside of the global pregnancy billing. Information entered in this section is not printed on the Prenatal Crystal report. If this section is used, either the Intake Note or the OB Master document is generated. The Reason for Visit panel on the Intake template displays the common reasons for a patient visit. You can select a reason for visit directly from the top list of common reasons for visit on the left pane. The Additional / Manage link opens the Reason For Visit template to access all reasons for visit. Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. OpenNextGen® Enterprise EHR. Open theIntaketemplate, and then expand theReason for Visitpanel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. To add reasons for the patient's visit with additional details by using another template, clear the Do not launch HPI checkbox, select the necessary reasons, and then select Save & Close . Patient's Reason for Visit

You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI check box and can select any HPI to open the corresponding HPI template. Note: You can configure sub menu items for reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the sub menu items appear
