Detailed Entry of Pregnancy Outcomes

You can enter a detailed entry in the Pregnancy Outcomes template. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN. The OBGYN Synopsis template opens. In the Pregnancy History section, select the Add/Update Detail button in the OBGYN Synopsis template. The Pregnancy Outcomes template opens. Add pregnancy outcome details in the Pregnancy history grid for each pregnancy. Detailed Entry/Edit on Pregnancy Outcomes The new data is included in the pregnancy outcomes detail auto-calculation, and it assumes Status (living now or deceased) and Delivery type (NSVD or C-section) are available. For a preterm or term pregnancy, Delivery Type is a required field. For a single gestation, it is not required to update the Multiple gestation birth order field. If the pregnancy is a multiple gestation: All fetuses from the multiple gestation belong to the same pregnancy, therefore the same Pregnancy # must be used for the outcome of each of the births. Documenting Pregnancy Outcomes

You can document the Pregnancy outcomes in the OBGYN Synopsis and Pregnancy Outcomes templates. Note: In a Gynecology patient, the OBGYN Details link would not be typically used in a pregnant patient. There are two ways to enter new pregnancy: Quick entry Detailed entry Quick Entry of Pregnancy Outcomes You can enter a quick parity entry in the Pregnancy Outcomes template. Detailed Entry of Pregnancy Outcomes You can enter a detailed parity entry in the Pregnancy Outcomes template. Manual Entry of Parity Values

For a disabled parity auto-calculation, you can either update the parity manually after entering in the Pregnancy Outcomes in the Pregnancy history grid or you can enter parity numbers without entering the details. The values added in the Pregnancy History grid appear in the Pregnancy Outcome template. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN. The OBGYN Synopsis template opens. In the Pregnancy History grid, enter the values in the Parity section. The G and P fields display the values as entered in the Parity fields. In the Pregnancy History grid, select the Add/Update Detail button in the OBGYN Synopsis template. The Pregnancy Outcomes template displays with red text on top of the template. Enter a Historic Pregnancy in the Pregnancy Outcomes Template

On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN. The OBGYN Synopsis template opens. Select the Add/Update Detail button in the OBGYN Synopsis template. The Pregnancy Outcomes template opens. If you enable the Parity auto-calculation setting, the Auto-calculation green text appears. Pregnancy Outcomes Select the Historic entry option in Pregnancy Entry Type field. Select the pregnancy number in the Pregnancy # field. Select a Full term or Premature pregnancy in the Gestational age section. For Full term pregnancy, if a pregnancy is >=37 weeks (37 weeks 0 days or later) the Full term field is incremented by 1. For Preterm pregnancy, if a pregnancy is <37 weeks (36 weeks 6 days or earlier) the Preterm field is incremented by 1 (Preterm = Premature) Note: Either a gestational age or the full term or preterm options must be documented for all outcomes with a gestational age >=20 weeks. Auto-calculation for Parity Values

The auto-calculation of parity enables you to make corrections to a patient’s parity if they are incorrect. In the Pregnancy History grid, without adding the details of each associated pregnancy, you can quickly document a patient’s current parity values. You can also view all patients within the practice that have disparate parity detail information. The following is auto-calculated in the NextGen® Adaptive Content Engine system: Gravidity is the sum of full term, abortion, and ectopic pregnancies and are automatically calculated. A current pregnancy if any is calculated in this count. Parity is calculated in TPAL format (T for total number of full term births; P for total number of preterm births; A for total number of abortions; and L for total number of living children). The system also further breaks down A to auto-calculate total number of spontaneous abortions, induced abortions, and ectopic pregnancies. Obstetrics

The configuration in the Parity detail auto calculation section applies only to the Obstetrics specialty. Obstetrics section The Parity detail auto calculation section is used to activate the auto-calculation of parity at the point of care when the pregnancy outcomes are entered. When the parity auto-calculations feature is turned off, the text "Parity auto-calculations not activated" appears. When you activate the Parity auto-calculate feature, the system adds a log in the OB Audit Log practice template. On the OB Audit Log practice template, you need to select the Parity detail auto calculation log type and the date range to view the items that have discrepancies in the parity numbers when compared to the pregnancy history grid entries. If the parity auto-calculation feature is turned on, the green text Auto-calculation appears on the Pregnancy Outcomes and OBGYN Synopsis templates.
