Note:
In a Gynecology patient, the
OBGYN Details
link would not be typically used in a pregnant patient.
There are two ways to enter new pregnancy:
Quick entry
Detailed entry
Quick Entry of Pregnancy Outcomes
You can enter a quick parity entry in the Pregnancy Outcomes template.
Detailed Entry of Pregnancy Outcomes
You can enter a detailed parity entry in the Pregnancy Outcomes template.
End a Current Pregnancy
You can end the pregnancy to stop a patient from being perpetually pregnant in the system.

Document the Pregnancy Outcomes Template
The Pregnancy Outcomes template provides you the ability to view and modify a patient’s gravidity and parity. You can view and modify the total parity values with or without the details of each pregnancy. Minimal required fields are Pregnancy Entry Type, Pregnancy #, and Delivery Type. It is preferable, on an OB initial visit to use the gravidity/parity fields and Pregnancy Outcomes templates from within the OB templates. When you activate parity auto-calculation, parity automatically calculates the details when you document historical pregnancy or end a current pregnancy When you select End current pregnancy the Gravidity reflects a non-pregnant status. This option is used on a postpartum or post-abortion visit and not on an initial prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN. The OBGYN Synopsis template opens. Select the Add/Update Detail button in the OBGYN Synopsis template. The Pregnancy Outcomes template opens. If you enable the Parity

Manual Entry of Parity Values
For a disabled parity auto-calculation, you can either update the parity manually after entering in the Pregnancy Outcomes in the Pregnancy history grid or you can enter parity numbers without entering the details. The values added in the Pregnancy History grid appear in the Pregnancy Outcome template. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN. The OBGYN Synopsis template opens. In the Pregnancy History grid, enter the values in the Parity section. The G and P fields display the values as entered in the Parity fields. In the Pregnancy History grid, select the Add/Update Detail button in the OBGYN Synopsis template. The Pregnancy Outcomes template displays with red text on top of the template. Updating parity manually In the Pregnancy Outcomes template, to enter a historic pregnancy: Select the Historic entry option in Pregnancy Entry Type field. Document the OBGYN Synopsis Template
The OBGYN link is available for female patients above the age of eight years. On the OBGYN Synopsis template, you can document information on the patient's menopause status, hysterectomy details, gynecological and pregnancy history, abnormal Pap, and contraception information. OBGYN Synopsis Note: The providers use OBGYN Synopsis only during a Gynecology visit and not during an OB prenatal visit. You can access the following templates from the OBGYN Synopsis template. Quick Entry of Pregnancy Outcomes
You can enter a quick entry or edit the information in the Pregnancy Outcomes template. In the OBGYN Synopsis or Pregnancy Outcomes template, enter parity values in the fields as per pregnancy details. You can enter the parity data or the outcomes data depending on the availability of the information and its pertinence to the pregnancy. Enter values in the Parity data fields. The Parity data fields are Full term, Premature, AB induced, AB spontaneous, and Ectopic. Quick Entry/Edit on Pregnancy Outcome Increasing any of these numbers by one adds a row to the Pregnancy history grid with this parity data listed, this new pregnancy outcome is included in the parity auto-calculation. Enter values in the Outcome Details data fields. The Outcome Details fields are Living now, NSVD, and C-section. This data is included in the pregnancy outcomes detail auto-calculation. Initial Prenatal Visit
The navigation tabs display based on which Obstetrics visit type. You can only change or select visit types from the Home or the Intake template. When you select the OB Prenatal – Initial or OB Prenatal visit type, the following navigation tabs display: Intake Gestational Age Histories Education Prenatal Detail SOAP Checkout Note: The above navigation tabs also display during nurse visits and subsequent prenatal visits. However, the Office Visit – OB, Virtual Visit Telephone, and Virtual Visit Video visit types display all the above navigation tabs except Gestational Age; the Chart Abstraction, Chart Update - OB, No-Show, and Consult visit types display all the above navigation tabs except Checkout. For the Patient Communication visit type the Telephone Call, Provider Test Action, Document Library, Order Management and Chart Abstraction displays. End a Current Pregnancy
When the outcome of a current pregnancy is unknown or not possible to follow up any longer, you can end the pregnancy to stop a patient from being perpetually pregnant in the system. Open a patient record where the pregnancy needs to be ended. On the Intake template, select Obstetrics for Specialty and then select Document Pregnancy Outcome for Visit Type. On the Histories template, expand the Pregnancy History panel and then select Add/Update. The Pregnancy Outcomes template opens. In the Detailed Entry/Edit panel, in the Pregnancy Entry Type section, select End current pregnancy. End current pregnancy on Detailed Entry/Edit panel Select the pregnancy number in the Pregnancy # field. In the Delivery type section, select Unknown (patient left the practice). Note: This option appears only if you have selected End current pregnancyoption. Select Add To Grid. The information appears in the Pregnancy history grid.
