Enter a Historic Pregnancy in the Pregnancy Outcomes Template
- On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.
The OBGYN Synopsis template opens.
- Select the Add/Update Detail button in the OBGYN Synopsis template.
The Pregnancy Outcomes template opens. If you enable the Parity auto-calculation setting, the Auto-calculation green text appears.
- Select the Historic entry option in Pregnancy Entry Type field.
- Select the pregnancy number in the Pregnancy # field.
- Select a Full term or Premature pregnancy in the Gestational age section.
- For Full term pregnancy, if a pregnancy is >=37 weeks (37 weeks 0 days or later) the Full term field is incremented by 1.
- For Preterm pregnancy, if a pregnancy is <37 weeks (36 weeks 6 days or earlier) the Preterm field is incremented by 1 (Preterm = Premature)
Note: Either a gestational age or the full term or preterm options must be documented for all outcomes with a gestational age >=20 weeks. An application message displays for the Add to Grid and Update buttons if not documented. The data entered remains but the entry to the grid cannot be saved until this is documented.
- If a pregnancy is < 20 weeks (20 weeks 0 days or later) the Delivery type increments the associated field by 1.
- If you select the abortion and ectopic fields, < 20 weeks fields to increment the associated full description detail fields on the right in the gray banner. The missed abortion is counted as a spontaneous abortion in the header.
- Select the Delivery type.
- Select Add to Grid button.
An automatic UDS message displays to add the baby’s ethnicity and race for UDS reporting.Note: The UDS Reporting message appears only if you select the End current pregnancy option and have enabled the Enable UDS Reporting setting in the UDS Reporting section in the Practice Configuration practice template. When you select an option in the Pregnancy Entry Type field, the Baby's Ethnicity/Race link displays above the Pregnancy history grid. If you do not select the Baby's Ethnicity/Race link, the automatic UDS message appears.
- Select Yes to document the baby's ethnicity and race in the Baby’s Ethnicity /race window.
- Select Save & Close.
- Select No, if you do not want to enter the baby's ethnicity details.
The entry displays in the Pregnancy history grid.
Review gravidity and parity auto-calculations related to the pregnancy outcome entries. The gray fields are a summary of most of the editable fields.
You can highlight the entry in the grid and edit the details, if required.
The Parity details are updated.
