Create Health Survey Reports

You can enter health survey details of a patient on the Health Survey Information window. Before you begin You must have NextGen® Enterprise Rosetta Interface Messenger to transmit the health survey report to public health agencies. Open NextGen® Enterprise EHR. Select a patient encounter, and open a template.For example, Intake. Select Navigation, and then select the Health Survey Info link.The Health Survey Information template opens. Health Survey Information To indicate the type of visit, select one of the following Episode of care options: Initial Patient Assessment Follow-up Visit In the Major Reason for Visit field, select a reason in the list. For asthma health survey reporting, do the following: In the Asthma problem field, select an asthma problem in the list. Note: The list includes the qualifying asthma problems documented in the Problems module or the Problems List panel. The Condition Control and Severity fields are enabled for documentation. Add Asthma SNOMED Codes for Health Survey

In run time, you must add the configured asthma code in the Problem List panel on the Histories template as an active asthma problem. The qualified active asthma problem is then available for selection in the Health Survey Information template for asthma problem survey reporting. In the Data Sets Setup practice template, click Data Set and select Asthma Codes. The existing data set items associated with the Asthma Codes data set appear in the grid. Data Sets Setup template Click Add. The Template Data Set Setup template opens. Template Data Set Setup template If applicable, click Diagnosis description to search and select a diagnosis. The associated diagnosis code automatically appears. If applicable, click Provider and select a provider from list. In Codes System, enter SNOMED. In Description, enter description of an asthma problem. In Code value, enter SNOMED code of asthma problem entered in the Description field. Click Save. Note: To add another item, click Clear For Add and repeat

Asthma Codes Data Set

The Asthma Codes data set enables you to configure the SNOMED code and description for asthma related problems. The configured asthma codes are used as the qualifying asthma problems for the asthma health survey reporting. Add Obstetric Problems

On the OB Problem Add template, you can add or update a patient’s obstetric (OB) problems, submit a problem to the Problems module, and move a problem to today’s assessment. The Associate Meds link enables you to view the active medications and associated diagnoses and update the diagnoses for an active medication. For more information on associated medications, go to NextGen Healthcare Success Community, and download the latest Getting Started User Guide forNextGen®Adaptive Content Engine. On the Prenatal Detail template, expand the OB Problems panel and select Add/Update. The OB Problem Add template opens. Select Add New. Automatically Flag Sensitive Encounters in the SNOMED® Code System

Before you begin To flag encounters as sensitive, you must select the Enable Sensitive Encounter checkbox on the Practice Configuration template. To add data elements for the code system, you must configure the Sensitive Encounter Configuration template. Open NextGen® Enterprise EHR. Select a patient record, and then create an encounter. In the left pane, select Problems. The Problems module opens. Select Add Problem, enter the code in the search result, and select Search. Add Problems in the Problem List

You can add a problem to the problem list of a patient. Open NextGen® Enterprise EHR. Open the Histories template. In the Problem List panel, select Add/Update. Enter a problem in the diagnosis search window, and select Search. Select the problem from the list of the search result, and select Select. Select the applicable ICD code, and select Select. The application automatically fills the associated values in the Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code fields in the Problem List panel. Note: For all new problems, the Mark as Chronic check box is selected by default. In Clinical Status, select the status of the problem. In Onset Date, enter the date of onset. In Resolved Date, enter the date when the selected problem is resolved. If the problem is not chronic, to mark the problem as a secondary condition, select Secondary Condition.
