You can enter health survey details of a patient on the Health Survey Information window.
Before you begin
You must have
NextGen® Enterprise Rosetta Interface Messenger to transmit the health survey report to public health agencies.
- Open NextGen® Enterprise EHR.
- Select a patient encounter, and open a template.
For example, Intake.
- Select Navigation, and then select the Health Survey Info link.
The
Health Survey Information template opens.
- To indicate the type of visit, select one of the following Episode of care options:
- Initial Patient Assessment
- Follow-up Visit
- In the Major Reason for Visit field, select a reason in the list.
- For asthma health survey reporting, do the following:
- In the Asthma problem field, select an asthma problem in the list.
Note: The list includes the qualifying asthma problems documented in the Problems module or the Problems List panel.
The Condition Control and Severity fields are enabled for documentation.
- To view a list of qualifying asthma problems, select information
.
Note: If the qualifying asthma problems are not available for selection, an alert message appears on the template.
- In the Condition Control field, select a condition of the problem from the list.
- In the Severity field, select the severity level of the problem.
- Select Save & Close.
