Patient Encounter Documentation

You can use NextGen® Adaptive Content Engine templates to document patient encounters on the visit types associated with the specialties available. The standard office and virtual visit workflow are documented on the following templates: Intake Histories SOAP Finalize Checkout Note: The workflow may vary with some specialties such as ophthalmology, cardiology, and obstetrics. For more information on the template set used to document a specialty, go to NextGen Healthcare Success Community and download the associated user guide. Some common features available in a standard visit template set are the Patient Information Card, panel controls, and links from the navigation bar. Using the Intake and Histories templates, a nurse or medical assistant can document the reasons for visit, medication, allergies, social history, and so on. Care Guidelines Report

You can generate a care guidelines report for the patient's record. You can use the ngsprep_care_guidelines stored procedure to generate enterprise-level reports that display all patient records (in alphabetical order) that are due for the specified care guideline or test. For example Pap, lipid panel. The ngsprep_care_guidelines store procedure, when runs, temporarily updates care guidelines data prior to run the report. It ensures that the generated report shows records with updated care guideline data to view all patients that are due for the selected guideline. However, the patient's chart is not updated. CAUTION: Care guidelines in the patient’s chart only gets updated when you create an encounter and run the care guidelines store procedure, for example on the Intake template. To create a specific guideline report, you need to filter the data by accessing the options from the NextGen Report Filter: Patient template. You can configure a memorized report. Histories Tab Links

The Histories template includes the following links: Demographics - The Demographics link displays the Patient Demographics template to enable you to document patient information such as contact information, address details, Primary Care Physician (PCP) details, insurance or pharmacy details, provider details, emergency contact details, reminders, and employment details. Order Management - The Order Management link displays the Order Management template, which enables you to update the applicable order details. Document Library - The Document Library link opens the Documents and Letters template. The Documents and Letters template includes links that generate the formatted letters and assessment templates, enables you to enter the applicable data. Document General Information

After reviewing the available patient details on the Home Page template, you can document the general information of a patient in the General panel. Open NextGen® Enterprise EHR. Open the Intake - Cardiology template. Click the General panel. Select the patient. If someone else is providing the patient details, select the relationship of the person with the patient in the Historian field. To capture the summary of care record information for a patient, click Transitioning into Care, and fill the information in the resulting window. To show the HCC links, badges, and grid columns on the templates, select Enable HCC for this patient. To indicate the disability, select Disability. Manage Pediatric Screening

You can document and manage the screening tests and results for a patient using the screening buttons in the Screening panel on the Pediatric SOAP template. Open the Pediatric SOAP template. To document the screening instrument, click the appropriate button from the Screening panel. To document the vision acuity details and the color vision test results, click Vision and enter the details on the Vision Screening - PE template. To document the patient's audiometry test details, click Hearing and enter the details on the Tone audiometry - PE template. To document the details of the tuberculosis risk assessment and skin test, click PPD and enter the details on the Tuberculin Skin Test template.
