Open Obstetrics Home Page

If you start on the Home Page-Obstetrics template then Obstetrics is automatically selected as the specialty. If you start on the Home Page template, then you have to select Obstetrics as the specialty and a visit type to access the template set associated with the selected parameters. Open NextGen® Enterprise EHR. To open the Templates module on a patient encounter, do one of the following: On the Categories or Encounter History tab, select Open Template. On Top Toolbar, select Templates. On the main menu, select File > New > Template. The Select Template window opens. Select Medical Records . In the Show section, select All . Review of Obstetrics Details by Intake Staff

The intake staff begins the current encounter on the Intake template. They can review the patient details documented during the previous encounters on the Home Page - Obstetrics template. The Home Page - Obstetrics template displays the information documented by the intake staff on the Intake and Histories templates for the provider's review. View Information in the Gestational Age Template

The Gestational Age template header displays the patient’s gravidity and parity, the initial/working estimated date of delivery (EDD), the working estimated gestational age (working EGA), the blood type and Rh, and the risk level. You can only view this information and can edit only the risk level. The Diagnostics Detail link opens a template on which in-office official sonogram results can be documented, if needed. Obstetrics Histories Template

The Histories template contains nine panels: Pregnancy History Gravidity/Parity Since Last Menstrual Period Medical/Infection History Medical/Surgical/Interim General Family Social Genetic Screening/Teratology Counseling OB Histories template The Histories template displays gravidity/parity, initial/working EDD, working EGA, blood type and Rh, and risk level as in Gestational Age template. After a complete history is taken, use the Risk Factors link to document the risk level. Note: When documenting risk factors, the practice may decide to only indicate the positive items rather than select negative for all of the remaining items. In addition, putting the actual risk level must be done by the provider. This may be done on this template or on the Prenatal Detail template. Documenting Information in the OB Gestational Orders Template

You can access the OB Gestational Orders template from thePrenatal Detail > Order Labs > OBGYN Lab Master > Labs > Orders by Gestational Week. The lab results fills the Order Management template, Lab Results - Obstetrics template, the Order module, and the master document. Before using the OB Gestational Orders template, you have to select the OB Prenatal- Initial visit type and enter the applicable information in the Gestational Age tab. Document the Pregnancy Outcomes Template

The Pregnancy Outcomes template provides you the ability to view and modify a patient’s gravidity and parity. You can view and modify the total parity values with or without the details of each pregnancy. Minimal required fields are Pregnancy Entry Type, Pregnancy #, and Delivery Type. It is preferable, on an OB initial visit to use the gravidity/parity fields and Pregnancy Outcomes templates from within the OB templates. When you activate parity auto-calculation, parity automatically calculates the details when you document historical pregnancy or end a current pregnancy When you select End current pregnancy the Gravidity reflects a non-pregnant status. This option is used on a postpartum or post-abortion visit and not on an initial prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.
