History Panel for the Plan/Intervention Tab

The History panel lists all the nursing care plans created for a patient. The nursing care plans can be created on the following locations: Nursing Care Plan History panel on the Care Coordination Home page Nursing Care Plan History panel on the Comp Assessment tab History panel on the Plan/Intervention tab Nursing Care Plan History The History panel for the Plan/Intervention tab functions exactly like the Nursing Care Plan History panel for the Care Coordination Home Page. Document Recent ER Visits (in last 12 months) Panel

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. If a patient has had any emergency room (ER) visits since his or her last visit or in the past 12 months, document those ER visits on this panel. On the Comp Assessment tab, expand Recent ER Visits (in last 12 months) panel. ER Visits Click the Location field and select the hospital the patient was admitted to. Click the Date field and use the calendar to select the date the patient was admitted. Click the Diagnosis field and type a diagnosis code for the patient. Note: You can double-click the Diagnosis field to launch the Diagnosis search template. Use the template to search and select the diagnosis for the patient. Click the appropriate option for Summary report received. Click Add. The entered record appears in the Recent ER Visits panel. Remove an ER Visit Parent topic: Comp Assessment Tab

Comp Assessment Panels

The Comp (Comprehensive) Assessment template consists of the following panels: Panel Description General Panel The General panel displays the information documented in the Care Transition template. Recent ER Visits Panel Any recent emergency room visits by the patient are recorded on this panel. Recent Hospitalizations Panel If the patient had experienced any recent hospitalizations, then you would enter that information on this panel. Problem List Panel The Problem List panel displays a list of the patient's chronic and secondary problems as documented in the Problem List after Discharge panel on the Care Transition template. Active and Manage Problems The Active and Managed Problems panel is where active and managed problems are entered and monitored. Nursing Care Plan History The Nursing Care Plan History panel lists all the nursing care plans that have been created for a patient. Patient/Caregiver Concerns Panel

Comp Assessment Problem List Panel

The Problem List panel displays a list of patient's chronic and secondary problems documented in the Problem List after Discharge panel on theCare Transition template. You can make changes in any of the documented problems by highlighting a problem in the grid and clicking the Edit button. The Add button enables provider to add any new problem.
