Comp Assessment Panels

The Comp (Comprehensive) Assessment template consists of the following panels: Panel Description General Panel The General panel displays the information documented in the Care Transition template. Recent ER Visits Panel Any recent emergency room visits by the patient are recorded on this panel. Recent Hospitalizations Panel If the patient had experienced any recent hospitalizations, then you would enter that information on this panel. Problem List Panel The Problem List panel displays a list of the patient's chronic and secondary problems as documented in the Problem List after Discharge panel on the Care Transition template. Active and Manage Problems The Active and Managed Problems panel is where active and managed problems are entered and monitored. Nursing Care Plan History The Nursing Care Plan History panel lists all the nursing care plans that have been created for a patient. The Active and Managed Problems panel is where active and managed problems are entered. Active and Managed Problems Clicking in any field launches the diagnosis search window. The Nursing Care Plan History panel lists all the nursing care plans created for a patient. Care Transitions Panels

The Care Transitions tab consists of: Panel Description General Use this panel to enter a patient’s general care transition information, including the primary care physician, case manager, and the facility admitted to or discharged from. Discharge Diagnosis Enables you to enter a diagnosis for a patient that can be used for continuity of care if the patient is being transitioned to another provider. The Discharge Diagnosis panel documents the diagnoses information after the patient is discharged from the hospital/facility.
