Comp Assessment Panels

Panel	Description
General Panel	The General panel displays the information documented in the Care Transition template.
Recent ER Visits Panel	Any recent emergency room visits by the patient are recorded on this panel.
Recent Hospitalizations Panel	If the patient had experienced any recent hospitalizations, then you would enter that information on this panel.
Problem List Panel	The Problem List panel displays a list of the patient's chronic and secondary problems as documented in the Problem List after Discharge panel on the Care Transition template.
Active and Manage Problems	The Active and Managed Problems panel is where active and managed problems are entered and monitored.
Nursing Care Plan History	The Nursing Care Plan History panel lists all the nursing care plans that have been created for a patient.
Medications Panel	This panel is documented in a variety of NextGen Adaptive Content Engine documents, including Intake Office Visit Workflow in NextGen Adaptive Content Engine and a number of the specialty user guides.
Social Panel	On the Social panel, you can record data on the patient, including tobacco use, alcohol and caffeine usage, race, ethnicity, languages, country of birth, educational levels, employment, military experience, marital status, social support, housing, and others.
Review of Systems Panel	The Review of Systems panel enables you to document the findings after reviewing a list of systems.
Patient /Caregiver Concerns Panel	You can record any concerns expressed by the patient, the patient's family or any caregiver on this panel.

Document Recent ER Visits (in last 12 months) Panel

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. If a patient has had any emergency room (ER) visits since his or her last visit or in the past 12 months, document those ER visits on this panel. On the Comp Assessment tab, expand Recent ER Visits (in last 12 months) panel. ER Visits Click the Location field and select the hospital the patient was admitted to. Click the Date field and use the calendar to select the date the patient was admitted. Click the Diagnosis field and type a diagnosis code for the patient. Note: You can double-click the Diagnosis field to launch the Diagnosis search template. Use the template to search and select the diagnosis for the patient. Patient's Social History

You can document a patient's social history. You can open the individual social history template or the Adult Social History template to document the social history information of a patient. The Adult Social History template includes the existing social history templates arranged in a one-page panel format. Each panel includes the data fields from the respective social history template. You can choose to document the social history information of the patient on individual social history templates or on the Adult Social History template. Document the Patient's Social Support Information

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. From the list, select Comp Assess - Social Hx, and then select OK. In the Living Situation panel, document the patient's current living details such as address, residential treatment, household composition, and risk assessment. In the Family Information panel, document the details about the patient's family members, family history of mental illness or substance addiction, and the emotions attached to the family members. In the Development panel, document the patient's motor and sensory development history. In the Abuse/Sexual Risk Behavior panel, document the details about any physical, sexual, or emotional abuse, the patient has experienced. Plan/Intervention Tab

You can define various team members who are going to provide healthcare services to a patient and maintain patient circumstances using the Plan/Intervention tab. The Plan/Intervention tab consists of the following panels: Panel Description Care Coordination Team This panel enables you to define the various team members who are going to provide healthcare services to a patient. When defining team member, you identify the individual provider and the agency with which the provider is associated.
