Record Recent Patient Hospitalizations

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Comp Assessment tab, expand Recent Hospitalizations panel. Click the Location field and select the hospital the patient was admitted to from the resulting list. Click the Admit date field and select the date the patient was admitted to the hospital from the resulting calendar. Click the D/C dates field and select the date the patient was discharged from the hospital from the resulting calendar. Click the D/C diagnosis field, and then select a discharge diagnosis from the window that appears. Click the appropriate option for Summary report received. Click Add. Comp Assessment Panels

The Comp (Comprehensive) Assessment template consists of the following panels: Panel Description General Panel The General panel displays the information documented in the Care Transition template. Recent ER Visits Panel Any recent emergency room visits by the patient are recorded on this panel. Recent Hospitalizations Panel If the patient had experienced any recent hospitalizations, then you would enter that information on this panel. Problem List Panel The Problem List panel displays a list of the patient's chronic and secondary problems as documented in the Problem List after Discharge panel on the Care Transition template. Remove a Diagnostic Study from Reason for Visit using the History Summary Panel

You can remove the studies reviewed record from the Reason for Visit panel using the History Summary panel. You can remove diagnostic studies from the Reason for Visit panel using the History Summary panel. Open NextGen® Enterprise EHR. From the History Summary panel, select Diagnostic. Select Add/Edit. The Diagnostic History Entry template opens. Select a diagnostic study that is previously added to the Reason for Visit panel. The Remove from HPI Grid button is enabled. Select Remove from HPI Grid. The Studies Reviewed row no longer appears on the Reason for Visit panel. Note: Removing the record from the Reason for Visit panel does not remove it from the grids on the Diagnostic History Entry template or the History Summary panel. Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty impacted by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is impacted by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, click the Transitioning Into Care link. The Transitioning Into Care template opens. Transitioning Into Care On the Transition into Care Management panel, click the Discharged field, and select the discharged date from the calendar. Click the Initial Communication field, and select the appropriate option from the list of communication types. Click the on field, and select the date of the initial communication from the calendar. Note: Notice that a appears to the right side of the initial communication on field, which reminds you to apply the CPT codes for Transitioning Into Care.
