Open the My Custom Plan template. At the bottom of the My Plan panel, click the Disposition field to select a value from the list or click the top, blank row to manually type the patient’s disposition in the field. View Referrals by Provider or Location

When Referrals is selected from the Order Options section, the grid View defaults to Providers. Open the My Custom Plan template. On the My Plan panel, select a diagnosis from the grid. In the Order Options section, select Referrals. In the Provider column, Show Internal may be the top option; leave the option to only see internal provider names. Note: The practice can set the default display to Allon the My Custom Plan Setup practice template, General Configuration panel by selecting the Set Default Display for Plan Type in My Custom Plan link. To view the names of every provider, select Show All, select a physician's name, and then set the Reason for Referral and Timeframe as appropriate. View Order Sets with a Referral

Open the My Custom Plan template. With the diagnosis highlighted, go to the Order Options section to choose the Plan Type of Order Set. Significance of Indicator Color

The color of the circle indicators on the Diagnosis Selection, Ordering Assessment, Diagnosis Group, and Order Set fields shows the state of the element on the My Custom Plan template. The colors of the indicators identify the status of the selections: Diagnosis Selection- black Ordering Assessment: No row selected - red indicator with None selected text Diagnosis is highlighted in the grid - green indicator with the diagnosis code and description text Diagnosis Group: Diagnosis is associated to a group name - the name displays and the indicator is green No associate group associated to a diagnosis - Mapping required red link displays and the color of the indicator is yellow My Plan Order Set: Diagnosis Group is associated to an Order Set - green No Diagnosis Group is available - the label displays as Dx Group required and the indicator is yellow No order sets available - a yellow indicator with a red alert label: No Available Order SetsMy Plan Diagnosis is associated to more than one

Enter and View Diagnoses on My Custom Plan

Orders can only be created after an assessment is designated for the encounter. On the My Custom Plan template, My Plan panel, do one of the following actions: Next to Diagnosis Selection, select the desired search option (Today's Assessments or Clinical Problems) to highlight the assessment in the grid. Click the Dx Search link to open the diagnosis search window. Search for and highlight the correct diagnosis. Click Select to return to My Custom Plan. Click Add/Edit Assessment or Add/Edit Problems below the grid (depending on the search option selected) to add as many assessments and/or clinical problems as is required. Click Save & Close to return to My Custom Plan. Note: There are five diagnoses or four clinical problems immediately visible in the grid with the option to scroll down to view all of the entered data. My Plan Sub-Navigational Link

The My Plan sub-navigational link opens the My Plan or My Custom Plan template to select an assessment and place the applicable orders associated with the diagnosis categories for the selected assessment. The template that opens in the My Plan tab out-of-the-box is the original template. Note: The My Plan tab can hold one of two templates: My Plan or My Custom Plan. My Plan - is packaged as the default template; recommended for those clients who wish to continue using the current My Plan template. This template will eventually be retired (date to be determined) and no enhancements are being applied to it. My Custom Plan – created to improve usability by reducing the number of clicks required for ordering while maintaining the functionality for Order Sets; recommended for clients who wish to take advantage of the significant modifications in the areas of Orders and Referrals.
