Flexible Care Management

The efficient and reliable coordination of medical care among multiple providers is an essential component in the delivery of safe and effective healthcare services to an individual patient. This is especially true when treating patients with chronic conditions such as diabetes or hypertension and those challenged by multiple illnesses who may struggle to negotiate the healthcare system or grasp the complexities of their own healthcare needs.

The primary goal of any Care Management effort is to ensure that the patient and all providers are working in concert to provide the best possible care without over or under treating the patient. To meet this goal, a care coordination effort must comprehensively define, coordinate and track in a single location all of the treatment and intervention efforts made for a patient by all providers involved in the patient’s care.

Healthcare cannot be of highest quality if it is not delivered in a well-coordinated and well-planned manner. When care is properly coordinated—with accurate transmission of information, effective communication, and appropriate follow-up care—patients who see multiple physicians and care providers experience fewer medication errors, have reduced hospital readmissions, and avoid frequent emergency department visits.

The Care Management User Guide for NextGen® Adaptive Content Engine explains how to use the Care Management features of NextGen® Adaptive Content Engine to create a comprehensive patient nursing care plan, review and incorporate into the nursing care plan evidence-based care guidelines, coordinate the activities of the various providers required to execute the nursing care plan, track the progress of the patient across the life of the plan while documenting the effectiveness of each treatment option.

Nursing Care Plans found in the Care Management suite neither follow the USCDI Care Plan certification requirement nor share to the Consolidated Clinical Document Architecture (C-CDA). If you need the care plan data to be shared through C-CDA , then you need to document it on the Care Plan template.
