Patient Care Plans

You can create a care plan to address patients' health concerns and monitor their progress toward resolving specific concerns. On the Care Plan template, you can do the following: Define and maintain a care plan for a patient that addresses health concerns, goals, interventions, and outcomes Record the functional and cognitive status of the patient Define, track, and review interventions by multiple providers and agencies Track the overall progress of the patient toward achieving defined goals The Care Plan template also enables you to meet the 2015 edition certification criterion, which requires a recording, changing, accessing, creating, and receiving care plan information. Telephone Communication

You can view and document the communication between a practice and a patient or a patient's representative. Depending on your preference, you can view and document telephone interaction details on the Telephone Call or Telephone Communication template. With the Telephone Call template you can document the reason for a call, add additional comments, and create tasks to assign to a patient's task list. You can access the template from the Template module. Telephone Call With the Telephone Communication template, you can quickly document details of telephone interactions with patients. The template includes the communication component that works seamlessly with NextGen® Adaptive Content Engine to streamline the documentation process. You can access this template from the Template module for all specialties. NextGen® Adaptive Content Engine ships with the Telephone Call template as the default. Consolidating Care Team Member Information

You can consolidate a list of care team members for a patient on the Care Team Members template. The patient care team can include clinical professionals, clinical pharmacist, agencies, organizations, relatives who act as care givers, and any other individual involved directly or indirectly with the care of the patient. These consolidated details in the Care Team Members template provide a comprehensive and up-to-date list of a patient’s care team members and meet the Stage 3 Quality Reporting Certification for 2017. The list includes patient care team members who are entered throughout NextGen® Adaptive Content Engine and the members added using the NPI Registry Search window. However, members added using the Care Team Members template only appear on the Care Team Members template and are shared with the Patient Care Team panel on the Care Plan template. You can use the Patient Care Team Members panel to view and manage a list of providers, agencies, organizations, care takers, and other individuals involved in the ongoing care of the patient. You can create and update Patient Care Team details, either from the Patient Care Team Members panel of the Care Plan template or from the Care Team Members template. From the Patient Care Team Members panel, you can: Create and update Patient Care Team. Add, update, and remove members and their details from the Patient Care Team. Practice Care Team

Practice care teams consist of clinical professionals and agency/organizations who are assembled to provide team care for patients. A practice would typically have multiple care teams defined to address specific needs and you can import the practice care teams to a patient care team. Prior to importing practice care teams to a patient care team, you must configure the practice care teams using the Practice Team Configuration template accessible. When you have configured the practice care team, you can import the practice care team to a patient care team using the Care Team Members template. When you import a practice care team to a patient care team, all the members of the practice care team will become the members of the patient care team in bulk.
