Documenting Vital Signs

You can document and update a patient's vital signs on the Vital Signs template. You can edit or remove a vital signs record from the Vital Signs grid. The vital signs template names vary based on a patient's age. For an adult patient, the template name is Adult Vital Signs. If a patient is younger than two years, the template name is Pediatric Under 2 Years Vital Signs. If a patient is two years or older, the template name is Pediatric Over 2 Years Vital Signs. You can change a pediatric patient's age to an adult age on the Practice Configuration template. A patient's height and weight fields are used to calculate their Blood Mass Index (BMI). BMI is not available for patients who are younger than two years. You can configure the normal vital signs range. If a patient's vital signs are outside the normal range, a message appears. Document the Patient Pre-Procedure Prep Details

Click Prep Type and select an appropriate prep type from the list. If the options are not appropriate for selection, double-click the top blank row in the list and enter the prep type in the Prep Type field. Enter any Comments on the pre-procedure prep given to the patient. You can use the My Phrases link at the top-right of the Comments field to add comments. The Clear button clears the data from the fields. To sort the phrases in the list, select the Summary or Phrase option and then click the My Phrases link. Click Add. The data appears in the grid. Vital Signs Monitor

The nurse or medical assistant has to document patient's vitals every three or five minutes during a procedure. To enable the nurse to document the vitals and at the same time assist the provider in performing a procedure, the vital signs interface is used. The vital signs interface captures the patient's vital signs from the vital signs monitor or vital signs device throughout the procedure at specific intervals of time. The captured vital signs import from the interface into the grid in the Vital Signs Monitoring panel. Add a Patient Prep

Click Prep type and select the pre-procedure prep type given to the patient. To manually enter the prep name, click the Prep type field and select the top-blank row in the list. If the list options are not appropriate for selection, click the top-blank row in the list and enter the prep name manually in the Prep type field. For Prep completion, select the status of the prep. Select Last solid intake date. To enter the Last solid intake time, do one of the following: Click the Last solid intake time field and enter the time. Click the clock to add the current system time. Click Last liquid intake date to select a date. To enter Last liquid intake time click the clock to add the current system time in the Last liquid intake time field. Click Prep results and select the appropriate result after patient prep. To enter additional information on the patient prep, click Comments. Patient Vital Signs

You can document and manage a patient's vital signs. You can add various vital signs for a patient during an encounter. The most recent value for each vital sign appears in the grid. A number appears adjacent to the vital sign to indicate the number of times a value is recorded for the vital sign. You can point to the number to view the values and the time when each value is recorded. Vital Signs Section You can view only the vital sign records documented for the current day in the grid. To view the historical vital signs records, you can use the History link. To view the vital signs graphs you can enable the Graph link. If the vital sign interface is installed, you can use the Import and Refresh buttons to open the VitalSignsSpotMonitor interface. View Patient Vital Signs

You can view a quick record of the patient's vital signs in NextGen® Enterprise EHR. Open NextGen® Enterprise EHR, and select a patient from the Patient Lookup window. To view a quick list of the patient's recorded vital signs, on the Quick Buttons panel, hover over Vital Signs. If no vital signs are recorded for the patient, a message appears stating that no vital signs records are found. The Vital Signs table appears based on patient's current age displaying the most recent sets of vital signs that are recorded for the patient in reverse chronological order. Note: Abnormal values are denoted by red-colored text and the indicator. To open the Vital Sign History window, which displays separate grids for the patient's vital signs and orthostatic vital signs, on the Quick Buttons panel, select Vital Signs.
