Enable the PRAPARE® link in the Social and History Summary Panels
To access the PRAPARE® form from the Social panel or the History Summary panel, you must configure the Practice Configuration template. From the File menu, click System\Practice Template. The Select Template window opens. From the Practice tab, select All. A list of practice-level templates opens. Select Practice Configuration, and then click OK. The Practice Configuration practice template opens. From the PRAPARE® section, select PRAPARE®. Click Save and Close. Enable Practice-Defined Questions on the PRAPARE® Form
To enable practice-defined questions on the PRAPARE® form, you must first configure the Practice Configuration template. From the File menu, click System\Practice Template. Document a Patient’s Social Determinants on the PRAPARE® Form
You can document a patient's social determinants in the PRAPARE® assessment tool. If the patient declines to perform the assessment, you can document that as well. When you select the Decline PRAPARE® check box, all documented details, except the automatically filled information, will be cleared. If you continue to document details after declining, the Decline PRAPARE® check box will be cleared. Open the PRAPARE® Assessment Tool template. To change the date, select in the Performed Date field. By default, the encounter date appears in this field. You can only enter the current or a past encounter date. To view the PRAPARE® details entered in the previous encounters, select the PRAPARE® Flowsheet link. To edit the patient's personal information, select Personal Information. By default, the patient's basic demographic details are filled from the Modify Patient Information template. PRAPARE® Assessment PRAPARE® Assessment You can document information related to a patient’s social determinants of health from the Protocol for Responding to and Assessing Patient's Assets, Risks, and Experiences (PRAPARE®) form on the PRAPARE® Assessment Tool template. Providers and health centers can analyze and act on the collected data as part of the national effort to better understand the socio-economic drivers that lead to poor health outcomes and increased clinical care cost for the patients. The PRAPARE® form presents questions specific to the following areas: Personal characteristics Family and home Money and resources Social and emotional health Practice defined information Based on a patient’s response to the questions, the system calculates and displays the score and interpretation (risk level) associated with the social determinants of health data of the patient.
