Histories Template

The Histories template enables you to document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history. The panels available in the Histories template are as follows: Section Description Problem List You can view, add, modify and retrieve patient's clinical problems and chronic conditions. Medical/Surgical/Interim You can document the patient's problem list and social, medical or surgical, diagnostic study, and family histories. Diagnostics Studies You can document the patient's diagnostic tests and its associated results. Family You can document the family history of the patient. Social You can document the social history of an adult or a pediatric patient. The categories in the social history includes tobacco/vaping use, alcohol/caffeine, lifestyle, occupation and so on. Document Dental Care Details of Pediatric Patients

You can document dental care for a pediatric patient in the Social panel on the Histories template. The options available to document dental care for a pediatric patient change based on the pediatric age group. These templates enable you to document age-appropriate nutrition information for pediatric patients. Open NextGen® Enterprise EHR. Open the Histories template. Expand the Social panel and select Nutrition. Click Add/Edit. Based on the patient's age, one of the following windows opens: Interval History - Nutrition birth- 12 mo for 0 to 12 months Interval History - Nutrition 1 to 4 yr for 1 to 4 years Interval History Nutrition Over 5 yr for 5 years or older To review the existing CQM – Dental Caries details of the patient, click the quality icon. Add Social History

You can add the social history of a patient in the History Summary panel on the Intake template and in the Social panel on the Histories template. Open NextGen® Enterprise EHR. To document the social history details, do one of the following: In the History Summary panel on the Intake template, select Social, select the relevant option in the list and Add/Edit. On the Histories template, expand the Social panel, select the relevant option, and then select Add/Edit. The social history template opens. Document the reported history information. Select Save & Close. Categories in Social History You can use different sections in the Social history to document the history of a patient. Patient's Tobacco Use Information You can document the patient's current or past tobacco use information. Document the Patient's Status You can document the status of a patient on the Social History - Status template. Screen for Future Fall Risk You can document the patient's risk for falls. Pediatric Confidential Information

You can use the confidential history templates to document a patient's confidential details. You can open the Peds Confidential History template by clicking the following links: The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. You can document the following confidential details of the pediatric patient: Alcohol and drug use, and any related comments from the historian on the Peds Confidential History - Substances template. Psychiatric history, child placement history, and incarceration details from the historian on the Peds Confidential History - Psychiatric template. Add Tobacco and Vaping Details

You can document the current tobacco use details and vaping details of a patient. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, select Tobacco/Vaping Use, and click Add/Edit. In the History Summary panel on the Intake template, click Social, click Tobacco/Vaping Use, and then click Add/Edit. Social History – Tobacco Expand the Tobacco Use panel. Note: For a patient whose age is 12 and above, the smoking status must be recorded to meet the Quality Reporting requirements. Select the appropriate options for the questions, and document the details in the necessary sections according to the tobacco usage by the patient. Document vaping details in the Vaping Use panel and enter any comments. You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Social History - Tobacco You can open the Social History – Tobacco template through the navigation bar, the History Summary panel on the Intake template, the Social panel on the Histories template, or the Risk Factors Config template.
