Add Social History

You can add the social history of a patient in the History Summary panel on the Intake template and in the Social panel on the Histories template. Open NextGen® Enterprise EHR. To document the social history details, do one of the following: In the History Summary panel on the Intake template, select Social, select the relevant option in the list and Add/Edit. On the Histories template, expand the Social panel, select the relevant option, and then select Add/Edit. The social history template opens. Document the reported history information. Select Save & Close. Categories in Social History You can use different sections in the Social history to document the history of a patient. Patient's Tobacco Use Information You can document the patient's current or past tobacco use information. Document the Patient's Status You can document the status of a patient on the Social History - Status template. Screen for Future Fall Risk You can document the patient's risk for falls. Document Pediatric Confidential History

You can document the confidential history of a pediatric patient. To open the Peds Confidential History - Substances template, do one of the following: In the Social panel on the Histories template, click the Confidential History link. In the History Summary panel on the Intake template, click the Confidential link. Peds Confidential History - Substances In the Substances section, enter the alcohol and drug history. In the Psychiatric/Placement section, enter the psychiatric history and past incarceration details. In the Sexual Practices section, enter past sexual practices, sexually transmitted infections, and comments from a parent, guardian, and/or the provider. In the Child Abuse section, enter details of the child abuse history and comments from the historian. Click Save & Close. Document Pediatric Psych History

You can document the pediatric psych history details. To open the confidential templates, do one of the following: In the Social panel on the Histories template, click the Confidential History link. In the History Summary panel on the Intake template, click the Confidential link. The Peds Confidential History - Substances template opens displaying the links to the confidential templates in the left pane. Click Psychiatric/Placement. The Peds Confidential History - Psychiatric template opens. If someone other than the patient is providing the details as a historian, then enter the historian details in Historian. Indicate the history of suicidal and homicidal thoughts. Indicate if the child has been diagnosed with a psychiatric problem. If you select Yes, then enter up to two diagnoses, onset age, and the patient treatment. Enter the name of the psychiatrist or therapist, and then type their phone numbers in the corresponding fields. Indicate the consent status for communication with PCP

You can document the pediatric incarceration details. To open the confidential templates, do one of the following: In the Social panel on the Histories template, click the Confidential History link. In the History Summary panel on the Intake template, click the Confidential link. The Peds Confidential History - Substances template opens displaying the links to the confidential templates in the left pane. Click Psychiatric/Placement. The Peds Confidential History - Psychiatric template opens. In the Incarceration section, indicate the history of incarceration. Document Pediatric Placement History

You can document the pediatric placement history. To open the confidential templates, do one of the following: In the Social panel on the Histories template, click the Confidential History link. In the History Summary panel on the Intake template, click the Confidential link. The Peds Confidential History - Substances template opens displaying the links to the confidential templates in the left pane. Click Psychiatric/Placement. The Peds Confidential History - Psychiatric template opens. In the Placement History section, indicate if the patient is currently placed out of home. Enter the date of current placement. Select the placement type, home type, and placement status. Indicate if the parental right has been terminated. Enter the residential caregiver's name, next court date, and jurisdiction. Document Pediatric Alcohol History

You can document the alcohol history of a pediatric patient. To open the Peds Confidential History - Substances template, do one of the following: In the Social panel on the Histories template, click the Confidential History link. In the History Summary panel on the Intake template, click the Confidential link. If the consent has been obtained to print the confidential details, select the Include confidential information in the pediatric Social History document with patient/guardian permission check box. In the Alcohol Use section, indicate if the patient drinks alcohol. If you select Yes or Formerly, then enter the type of alcohol, frequency of alcohol consumption, amount, and the details of the last drink. Indicate if the patient has sought a treatment for alcohol abuse. If you select Yes, then enter the date of last treatment for alcohol abuse and the number of treatments taken for alcohol abuse.
