Document Pediatric Drug History

You can document the drug history of a pediatric patient.

  1. To open the Peds Confidential History - Substances template, do one of the following:
    • In the Social panel on the Histories template, click the Confidential History link.
    • In the History Summary panel on the Intake template, click the Confidential link.
  2. In the Drug Use/Abuse section, indicate if the patient uses drugs. If you select Yes or Formerly, then do the following:
    • Document the drug type, frequency, and the route of taking drugs.
    • To indicate that the patient has quit taking a drug, select the Quit check box near the respective drug.
  3. Indicate if the patient has sought treatment for drug abuse.
    • If you select Yes, then enter the number of times the patient was treated and the date of the last drug treatment, then select the Outpatient or Inpatient check box to document the type of treatment.
  4. Indicate if the patient has involved in a 12-step program.
    • If you select Yes, then select a check box to indicate if the patient is involved currently or was involved previously.
  5. Indicate if the patient had emergency medical attention required due to drug use.
    • If you select Yes, then document the number of times patient was treated.
  6. Indicate the family history of drug use.
    • If you select Yes, then document the patient's relationship with the family members who has a history of drug abuse.
  7. Click Save & Close.
