Panels	Description
Provider Interactions	Use this panel to record interactions with other providers.
Telephone Interactions	Enter patient’s interactions over the phone on this panel.
Face-to-Face Interactions	Enter patient’s face-to-face interactions on this panel.
Care Coordination Interactions	Enter patient’s care coordination-related interactions on this panel.
Document	Use this panel to add or update the visit diagnosis codes, document the type of visit, submit the encounter visit code, and generate the visit note. The Document panel also contains links to generate several care management documents.

Plan/Intervention Tab

You can define various team members who are going to provide healthcare services to a patient and maintain patient circumstances using the Plan/Intervention tab. The Plan/Intervention tab consists of the following panels: Panel Description Care Coordination Team This panel enables you to define the various team members who are going to provide healthcare services to a patient. When defining team member, you identify the individual provider and the agency with which the provider is associated. Barriers to Care This panel enables you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan. History This panel lists all the nursing care plans that have been created for a patient. These plans can be created on the History panel. Interventions This panel lists the current interventions that have been defined for a patient along with the relevant data necessary to define the intervention. Progress This

Document Telephone Interactions

Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. On the Documentation template, expand theTelephone Interactions panel, and then click Add The Telephone Interactions template opens. Fill out all the fields that apply for this interaction. Click Save. Click Clear For Add to enter multiple actions or click Close . Note: Use the Edit and Remove buttons to manage Telephone Interactions. Parent topic: Documentation Tab

Record Face-to-Face Interactions

Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. On the Documentation template, expand the Face-to-Face Interactions panel, and then click Add The Face to Face Interactions template opens. Fill out all the fields that apply for this interaction. Click Save. Click Clear For Add to enter multiple actions or click Close. Note: Use the Edit and Remove buttons to manage Face-to-Face Interactions. Parent topic: Documentation Tab

Document Provider Interactions

Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. On the Documentation tab, expand the Provider Interactions panel, and then click Add The Provider Interactions template opens. Type a description of the action taken in the Actions field. Click Save. To add another entry, click Clear For Add and repeat steps 2 and 3. When you have completed all need entries, click Close. The template closes and the saved entries appear on the Provider Interactions data grid. To remove an entry from the Provider Interactions grids, click the entry to be removed and click Remove. To change an entry on the grid, click the entry to be changed and click Edit. The Provider Interactions template appears and you can edit the text in the Actions field. Use the Document Panel to Complete a Visit

Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. On the Documentation template, expand the Document panel. Document Add or update the diagnosis code as needed. Choose the appropriate type of visit for the encounter. Click the Add button. Generate the visit note by clicking the correct document link.
