Reason for Visit on the SOAP Template

Open the SOAP template. Do one of the following: If the Introduction field contains text, review the content and make any necessary changes. If the Introduction field is blank, compose an introduction for the patient. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If checked, clear the Do not Launch HPI check box. To add additional details to a previously entered reason for visit, click one of the reasons for visit in the data grid. The appropriate HPI template opens. To add a new reason for visit: Select a complaint from the reasons for visit in the left pane. In the Other field select a reason from the resulting list or type a reason for visit. Document the Review of Systems Findings

The Review of Systems panel allows you to document the findings after reviewing a list of systems. Note: To copy the previous review of systems information to the current encounter, click ROS History. The Historical Information window opens, click Copy ROS & Close to copy previous review of systems information to the current encounter on the SOAP template. On the Historical Information window you can view Encounter Date, Provider, Reason for Visit/HPI, Diagnosis and copy System and Findings, Comments and Narrative to the current encounter. You can search or filter for specific content on the Historical information window by using the picklist available for each field except Findings, Comments and Narrative. For Findings, Comments and Narrative, you must type minimum 3 characters to enable the Search button. Open the Intake template. In the Review of Systems panel, click Orthopedic - ROS. Documenting SOAP Information

Use the SOAP template to enter history, exam, assessment, and plan information. Reason for Visit on the SOAP Template You can use the Reason for Visit to enter the patient introduction and additional details about the reason for visit. Physical Exam Panel You can document physical exam details. Document Review of Systems Details You can review and update symptoms entered during the patient intake process in the Review of Systems section on the SOAP template. Document a Specific Complaint

After the intake staff completes their work with the patient, you can document the patient encounter. An essential part of the workflow is to complete the appropriate physical examination template. The grid functionality on the Dermatology physical examination (PE) templates enables you to document multiple complaints on a single PE template. On Dermatology Home Page template, review the patient's historical information documented in the previous encounters. On the Histories template, review the patient information and make the desired modifications. On the GET template, and add your findings using Graphical Exam Tool. On the SOAP template, view the details of reason for entered by the intake staff in the Reason for Visit panel. You can enter additional reasons for visit. You can document the findings of a patient's review of systems for multiple systems using a single template or a single system. Before you begin The One Page ROS option opens a gender-specific or gender-neutral Review of Systems (ROS) template based on the configuration of the Review of Systems section on the Framework Content system template. The Select other ROS option displays the practice's systems configured on the Framework Content system template. You can configure the gender-neutral Reproductive ROS template based on the configuration of the SOGI/Transgender section on the Practice Configuration template.
