Document a Specific Complaint

After the intake staff completes their work with the patient, you can document the patient encounter.

An essential part of the workflow is to complete the appropriate physical examination template. The grid functionality on the Dermatology physical examination (PE) templates enables you to document multiple complaints on a single PE template.

  1. On Dermatology Home Page template, review the patient's historical information documented in the previous encounters.
  2. On the Histories template, review the patient information and make the desired modifications.
  3. On the GET template, and add your findings using Graphical Exam Tool.
  4. On the SOAP template, view the details of reason for entered by the intake staff in the Reason for Visit panel. You can enter additional reasons for visit.
  5. In the Review of Systems panel, to document patient's signs and symptoms, do one of the following:
    • Click the ROS defaults field, and then select an appropriate default ROS from the list.
    • Click DERM - ROS and do one of the following in the Dermatology-ROS template.
      • To select negative or positive for symptoms in a system, select either Neg or Pos options.
      • To mark negative for all symptoms in a specific system, select All neg in the respective system.
      • To document the findings in detail for a specific system, click the respective system link, and then document the findings.
  6. On the SOAP template, in the Physical Exam panel, click Rash | Dermatitis if the patient is visiting for rash.

  7. In the Chief Complaint field, the select the reason for visit, for example rash.
  8. To enter the diagnosis, do the following:
    1. Click the Assessment field.
    2. Select a diagnosis.
      The details appear in the Assessment field.
    3. Click Status, and select a diagnosis status.
  9. Document the Exam, Treatment Today, Treatment to be scheduled, Medications, and Today's Exam details.
  10. Click Add.
    The system adds an entry in the Today's Exams grid and clears the template.
  11. Click Save & Close.
