Complete the Derm Quick HPI Template

The Derm Quick HPI template enables you to enter the following patient information:
  • General information about the patient's chief concern
  • Dermatology specific information, including location of concern, skin type, cancer history and date of last exam
  • Current and previous treatments
  • Additional relevant details

You can add up to six complaints on the HPI grid.

  1. In the Reason for Visit panel of the SOAP template, click derm quick hpi in the left pane.
    The Derm Quick HPI template opens.

  2. In the General panel, click the Concern field and select the patient's chief concern from the resulting list.
  3. To enter a duration for the concern, click the Duration field and use the resulting calculator.
  4. If this is a follow-up exam for treatment of the chief concern, click the Follow up check box.
  5. To select the current status of the concern, click Status.
  6. To indicate the date that the patient first noticed the concern, click Lesion noticed.
  7. If appropriate, select a Severity for the concern, either Mild, Moderate or Severe.
  8. Click the Quality field and select one or more descriptors from the resulting list to describe the concern. When you have selected all the applicable descriptors, click Close.
  9. If the patient is female verify with her the Pregnancy Status displayed. After verification, click the Status Verified check box.
  10. In the Dermatology Specific section, click the Location field and use the resulting lists to describe the location of the concern.
    If there are multiple locations, click the Additional Locations link and use the Derm Image Body template to indicate additional locations.
  11. To select the entry that best describes the patient's skin type, click the Skin Type field.
  12. If the patient reports a history of skin cancers, click any of the History of check boxes that apply.
  13. Click the No previous treatment check box In the Treatment section if the patient indicates that he or she has not had any previous treatments for the chief concern.
  14. If the patient has received treatment for the chief concern, click the Previous treatment/management field and select the appropriate treatment from the resulting list.
  15. Click the Response field and select the appropriate treatment response from the resulting list.
  16. If the patient is currently being treated for the chief concern, click the Current treatment field and select the medication used in the treatment from the resulting list.
  17. Complete the Additional Information section, if applicable.
  18. Click Add to Grid. The entered data appears as a record in the Concern grid.
    The chief concern picklist appears.
  19. Select another concern from the list or click Close.
  20. Click Save & Close to return to the SOAP template.
    The Review of Test panel shows information about pathology/diagnostic studies, lab orders/results, referrals, and other reports. You can select a test in the grid and click Add to populate information about the test to the Action Details/Notes section.

    The Derm Quick HPI template also includes a Skin Cancer History panel.
