Document a Medication Review for a Chronic Condition
The Medication Review template enables you to perform a manual or electronic medication reconciliation.
- On the Chronic Conditions - Hpi, click the Medication Review link to open the Medication Review template.
- To manually update medications, move them from the Medication List grid to the Medication Review grid.
Alternatively, an electronic medication reconciliation can be done using either a CCDA received by the practice or the electronic medication history query option.
- Click Electronic Reconciliation to open the Clinical Reconciliation template.
