Document a Medication Review for a Chronic Condition

The Medication Review template enables you to perform a manual or electronic medication reconciliation. On the Chronic Conditions - Hpi, click the Medication Review link to open the Medication Review template. To manually update medications, move them from the Medication List grid to the Medication Review grid. Alternatively, an electronic medication reconciliation can be done using either a CCDA received by the practice or the electronic medication history query option. Medication Reconciliation for a Patient

Medication reconciliation is a review of patient adherence to prescribed medications. Reconciliation can be performed by comparing two lists using the Electronic Reconciliation option or the Manual Reconciliation check box. This means you have a paper chart that you are comparing to the patient's current medication list, or you are documenting adherence to an existing medication list if there are no external sources to reconcile. Medication Review You can perform medication reconciliation in situations including, but not limited to, the following: New patients Periodically for established patients After a hospitalization or emergency department visit After care by another provider Whenever the provider thinks necessary The Medication Review panel contains two grids. The Medication List - includes a list of current medications that need to be reviewed for the patient adherence to the prescribed medication. Reconcile Medications

The Medications Review template enables you to reconcile medications listed for the patient with the medications the patient is taking and how the patient is taking them. The Medications Review template has three panels: Reconciliation type – has a check box that indicates if reconciliation was completed manually and has an Electronic Reconciliation button. To use electronic reconciliation, you must have a CCDA document or medication record to import. Medication Module– displays all medications that were added from the Medications module. Medication Review– displays current medications pending review in the Medication List and reviewed medications appear in the Medication Review grid. Open the Medication Review Template

You can access the Medication Review template to review the medication. Open NextGen® Enterprise EHR. On the Intake template, expand the Medications panel. To open the Medication Review template, select Reconcile. Review Medications Manually You can manually review the medications details that are taken by a patient for adherence to the prescribed medicines. Update the Adherence of a Medication You can modify a patient's adherence to medication. Reconcile Medications Manually

Use the Medications Review panel on the Medications Review template to reconcile medications manually. Note: If no manual reconciliation has taken place, the Manual Reconciliation check box in the Reconciliation type panel is unchecked; as soon as you reconcile any medication manually, the box is selected and fields open for you to enter additional data. Medication Review template Click the Reconcile button below the Medications grid to open the Medication Review template. If the patient is taking all medications in the Medication List grid as directed, click the Review All – Taken As Directed button to move them all into the Medication Review grid. If not, skip to step 3. Select the check box Review – adherence at the top of the Medication Review panel. The drop-down field selection defaults to “taking as directed”. In the Medication List, click on any medication that the patient is taking as directed. Medications that you select move to the Medication Review grid with that adherence

Medications

The Medications panel for Ophthalmic specialties displays a list of current medications documented in the system for the patient. You can filter medications by selecting one of the following options: All, Ocular, or Systemic. You can choose to display both current and inactive medications in the grid. If a patient is not currently taking any medications, select the No medications this encounter check box. You can document new medications and information for existing medications through the Medication module. medication panel Note: You must refresh the Medications grid to display updated information from the Medication module. If you document medication information in the Medication module, you will not see the new information until you refresh the grid.
