Document Lab Glucose Results

Chronic Conditions Data
The Chronic Conditions and Chronic Conditions HPI templates are used for documentation of multiple chronic conditions in one place, including diabetes. In addition, you can review health maintenance items, order tests, and review recommended care items and patient goals and/or self-management. Note: The Chronic Conditions and Chronic Conditions HPI templates cannot be used on the same encounter. To use the Chronic Conditions template, be sure that chronic conditions is not selected in the Reason for Visit panel. Enter Medication History on the Chronic Conditions - HPI
On the Chronic Conditions - Hpi template, navigate to the Medications section. Click History. The Medication History template opens. Document Patient Reported Glucose Results
On the Chronic Conditions - Hpi template, navigate to the Labs section. To document and graph the patient’s reported glucose results, click the Home Glucose Readings link. The Home Glucose Map template opens. To enter values, do one of the following actions: Click Download Meter Data. Click the Datefield to enter the client reported value and continue to enter data points in the resulting windows. To create a visual representation of the data, click the Graph Daily Readings and/or Graph Min., Max. & Average. Click Save & Close to return to the Chronic Conditions - Hpitemplate. Activate Diabetes Care Guidelines
Care guidelines can be set up for diabetes. The guidelines include tests, as well as recommended care items, and are a form of clinical decision support. Practices can customize care guidelines on the Activate Care Guidelines template. For more information on configuring care guidelines, go to NextGen Healthcare Success Community and download the latest Care Guidelines User Guide for NextGen® Adaptive Content Engine. On the Chronic Conditions – Hpi template, navigate to the Care Guidelines section and click Update. The Activate Care Guidelines template opens. Click Add/Updatein the Care Guidelines to add new guidelines or edit existing ones. Selecting the Diabetes navigational link in the Care Guidelines template displays the configured diabetic guidelines. Click OK to return to the Chronic Conditions - Hpi template. Once the clinical or care guidelines are configured, they can be viewed and updated on the Chronic Conditions – Hpi template. Parent topic: Access the Chronic Conditions -
