Chronic Conditions Data
The Chronic Conditions and Chronic Conditions HPI templates are used for documentation of multiple chronic conditions in one place, including diabetes. In addition, you can review health maintenance items, order tests, and review recommended care items and patient goals and/or self-management. Note: The Chronic Conditions and Chronic Conditions HPI templates cannot be used on the same encounter. To use the Chronic Conditions template, be sure that chronic conditions is not selected in the Reason for Visit panel. The Chronic Conditions and the Chronic Conditions HPI templates also provide access to the following areas: Flowsheets Health Promotion Plan Lab results and review Medication review and reconciliation Medication history Access the Chronic Conditions Template Access the Chronic Conditions - Hpi Template Parent topic: Diabetes Care

Chronic Conditions
You can view the chronic condition details of a patient from the Reason for Visit panel. The Chronic Conditions template enables you to document the chronic conditions for the patient. In addition, you can view and associate the patient’s active medications with three active diagnoses (that is, Diagnosis 1, Diagnosis 2, or Diagnosis 3) using the Associate Meds link available on the Chronic Conditions template. Document Chronic Conditions You can document patient's chronic condition details in the Reason for Visit panel. Associate Medications The Associate Medications template enables you to view and associate an active medication with up to three active diagnoses. Enter Medication History on the Chronic Conditions - HPI
On the Chronic Conditions - Hpi template, navigate to the Medications section. Click History. The Medication History template opens. Document Lab Glucose Results
On the Chronic Conditions - Hpi template, click the Lab Review link. The Lab Review template displays external and in-house lab results. View or enter lab results data. Click Save & Close to return to the Chronic Conditions - Hpi template. Parent topic: Access the Chronic Conditions - Hpi Template

Access the Health Promotion Plan
Do one of the following: Click the Health promotion plan link on the Chronic Conditions - Hpi. Click the Health promotion plan link on the Screening Tools template. For more information on Screening Tools template, go to the NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen Adaptive Content Engine.
