Provider CMS Measures

This panel provides the ability to pick and indicate exceptions for the measures. Open the Provider CQM Maintenance system template. Click the Provider field and select a name from the list. The default option in the System field is CMS. Click the Measure number field and select a value from the list. The Measure short description field automatically fills the information based on the selected measure. In the Default exception class field, click and select an option from the list. Click the Default exception description and select an option from the list. Click Add/Update. The record appears in the grid and displays confirmation text. Provider CMS Measures To delete a measure, select the desired row on the grid and click Remove. Remove an Exception Measure

Open the Provider CQM Maintenance template. Navigate to the Exception Maintenance panel. Type the exclusion name in the Search Measure field to find the exclusion measure that you want to remove and click search icon. Select the exclusion measure from the grid and click Remove. Import Exception Measures

Open the Provider CQM Maintenance template. Navigate to the Exception Maintenance panel. Note: The administrator should have read-only access to the folder that stores the csv. file. For more information on folder permission, go to Microsoft® website. Enter the location of the csv. file in the Import Path field and click Import. For example, the file name path <folder name>\<file name>. Exceptions_Measures\import_exception_data.csv The total number of records ready to be imported display on top right-side of the grid. Note: If the import is not successful, the system will display an error message. Remove is enabled at the time of importing the csv. file. Verify the import grid results with the spreadsheet. Default Effective Date populates as 01/01/1900. If a measure has multiple exclusion items (class, type and exceptions), then the Exception Class column shows as Multiple Exception Class. Configure Non-CMS Provider Measures

You can configure non-CMS Provider measures for CPT II codes. To enable a provider to document an exception and submit a CPT II code manually, you must assign a measure to a provider. Open the Provider CQM Maintenance template. Provider Measures (not CMS) Panel In the Provider Measures (not CMS) panel, select the Provider field. Select a provider in the list. Select the System field, and select Claims Based Measures. Note: The grid displays measures that have been set up in the Claims Based Reporting Setup panel on the Provider CQM Maintenance template that matches the System selection you have selected. Select the Measure # field, and select a measure in the list.The Measure short description field displays the description of the measure. Select Add.The Provider Measures (not CMS) configuration is added to the All Providers Measure grid. Viewing Exceptions and Measures

You can manage exceptions and view measure details on the Exceptions/Measures template. You can open the Exceptions/Measures template from the navigation bar or from the CPT II Claims Based Reporting panel on the Finalize template. You can view the following types of exceptions and measures on the Exception/Measures template: Registry-based measures Procedure coded measures Custom data mapping Exception/Measures For information on how to address specific measures, go to NextGen Healthcare Success Community, and download the latest guides for NextGen® HQM. Configure Measures for Claims-Based Reporting

On the Claims Based Reporting Setup panel, system administrators can add existing or newly-created measures to practices. On the Provider CQM Maintenance template, expand the Claims Based Reporting Setup panel. Claims Based Reporting Setup panel In the Practice(s) to View/Update section, do one of the following: To view or update measures for all practices, select the All option. To view or update measures for a practice, select the Selected option, and select a practice from the list. In the System field, do one of the following: To add an existing measure, select QID, click the Measure # field, and select a measure on the Dbp Provider Measure List window. Dbp Provider Measure List To add new claims-based measure, select Claims Based Measures. Select the Measure # field, and select <Add New> on the Dbp Provider Measure List window.
