Documenting Community Resources

You can document local resources and support recommended to the patient to aid in meeting the patients' psychosocial needs. You must first set up the community resources for your area on the Community Resources Setup practice template. For more information on the configuration, go to NextGen Healthcare Success Community, and download the latest Community Resource Setup Practice Template forNextGen®Adaptive Content Engine. The Community Resources template contains the All Resources and Patient's Resources grids. NextGen® Enterprise EHR fills the All Resources grid with data entered on the Community Resources Setup practice template. Configure Community Resources

Community resources configured on the Community Resource Setup practice template appear on the Community Resources template in the All Resources grid. Resource data can be added, updated, and removed using the fields above the grid. From the File menu, select System/Practice Template. Select the Practice tab and select the All option. Select Practice Configuration Template to open the Practice Configuration Template. Select Community Resource Setup in the navigation bar.The Community Resource Setup template opens. Community Resource Setup Enter the type of resource such as Pharmacy, Medical Office, and Dental in the Type field. Note: The system creates a DB picklist of the values entered here to be used for future entries. Enter the name of the resource in the Resource field. Use the Street, City, State and Zip fields to enter the address of the resource.The system concatenates the address details into the Address column in the grid. Record Community Resources Discussed with Patients

You can record the community resources details that you have discussed with patients. Open NextGen® Enterprise EHR. For a patient encounter, open a template. For example, Intake To open the navigation bar, select Navigation. Select Community Resources. The Community Resources template opens. Community Resources Review the available resources in the All Resources grid. To filter the grid by a specific type of community resource, in the Type field, select a value from the list. To add the resource to the Patient's Resources grid, do one of the following: In the All Resources grid, select the resource, and then select Include selected resource. Select the Automatically perform "Include" or "Remove" action on row select check box, and then in the All Resources grid, select the resource. The community resource item is added to the Patient's Resources grid. To view community resources discussed in previous visits, select the Prior visits option. Select Save & Close. Remove a Community Resource from the Patient's Resources Grid

On the SOAP template, navigate to the Assessment/Plan panel. Select Community Resources in the left pane to open the Community Resources template. Do one of the following: Highlight the desired resource and click the Remove selected resource arrow. Select Automatically perform "Include" or "Remove" action on row select and click the required resource in the All Resources grid. Open the PRAPARE Assessment Tool

You can open the PRAPARE® assessment tool from multiple locations. Before you begin To enable the PRAPARE® link in the Social panel of the Histories template and the History Summary panel of the Intake and SOAP templates, you must configure the PRAPARE® in the Practice Configuration template. Open NextGen® Enterprise EHR. Open a patient record and create an encounter. To open the PRAPARE® Assessment Tool template, do one of the following: Open the Intake template, expand the History Summary panel, and then select PRAPARE®. Open the Histories template, expand the Social panel, and then select PRAPARE®. On the SOAP template, expand the History panel, and then select PRAPARE®. In the Encounter History tab, select Open Template to open the Select Template window. Select the Medical Records tab. Under the Show section, select All.
