Add Child Care, Relationship, and Tobacco Exposure Details
You can add child care, relationship, and tobacco exposure details of a pediatric patient.
- To open the Pediatric Social History – Relationships template, do one of the following:
- On the Histories template, expand the Social panel, select Relationships, and then click Add/Edit.
- In the History Summary panel on the Intake template, click Social, click Relationships, and then click Add/Edit.
- In the Historian field, enter the relationship of the person who provides the patient information.
- In the Child Care section, select the provider and enter the number of days/week the child is with the child care provider.
- If you select Daycare, enter the facility name.
- If you select Relative, enter the relationship in the relative field.
- In the Relationships section, do the following:
- To add the primary residence details, click Add in the Primary section, and add the details with whom the patient is living with in the primary residence, then select the percentage of time spent in the primary residence in the Time spent field.
- To add the secondary residence details, click Add in the Secondary section, and add the details with whom the patient is living with in the secondary residence, then enter the percentage of time spent in the secondary residence in the Time spent field.
- To document the maternal depression screening, select Maternal depression screening performed and Maternal depression screening result discussed check boxes.
- Select the parents' marital status.
- Indicate if the parents had multiple birth.

- If you select Yes, then enter the birth order and number of births.

- If the child has any siblings, enter the number of siblings.

- Enter the birth order of the child and the relationship with the siblings.

- Indicate if the child cooperates with family/friends.
- Indicate if the child cooperates with teachers.
- Indicate if the child has enough friends of both sexes.
- Indicates if there are any concerns about the relationship with family, friends, or other.

- If you select Yes, then enter your concerns.
- In the Tobacco Exposure section, the Passive Smoke Exposure status is pre-selected based on the selection in the Passive Exposure panel of the Social History - Tobacco template.
- If Yes, then select No or Yes for Smoke outside only.
- Save and close the template.
