Edit a Reason for Visit on SOAP

After the intake staff documents the patient's reasons for visit, you can edit the reasons for visit on the SOAP template to document additional details. Open NextGen® Enterprise EHR. On the SOAP template, expand the Reason for Visit panel. To edit a reason for visit, do the following: Select the HPI that you want to edit in the Reason for Visit grid. Review Reasons for a Patient's Visit

You can review and update reasons for a patient's visit during the provider workflow. Open NextGen® Enterprise EHR. Open the SOAP, and then expand the Reason for Visit panel. Review and update the patient summary in the Introduction field. Review and update reasons for visit, diagnostic studies, and chronic conditions. To add any additional information, select Comments. Review Intake Comments on the SOAP Template Review comments entered during the intake if the Intake Comments link on the SOAP template indicates that content is present. Edit a Reason for Visit on SOAP You can edit existing reasons for visits on the SOAP template. Document the Patient's Reason for Visit as a Provider

The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. Copy Reasons for Visit from Previous Encounters

You can copy reasons for a patient's visit from previous encounters to the current encounter instead of manually adding them. OpenNextGen® Enterprise EHR. Open theIntaketemplate, and then expand theReason for Visitpanel. Select HPI History. The Historical Information - HPI window opens. Note: The records are displayed for the default duration configured in the Copy Previous Visit Information section of the Practice Configuration template. To filter the records in the grid, enter the search criteria inFilter by, and then selectSearch. Set Up HPIs to Appear in the Reason for Visit

You can set up the History of Present Illness (HPI) to appear in the reason for visit. A practice can configure the Reason for Visit panel using the Framework Content system template. For more information on the reason for visit configuration, go to NextGen Healthcare Success Community, and download the latest Framework Content System Template forNextGen®Adaptive Content Engine. Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. In the Show section, select All. To open the Framework Content template, double-click Framework Content. Select Specialty and select a specialty from the list. Expand the Reason for Visit panel. In Filter, select the all option. Select the reason for visit linked to the appropriate History of Present Illness (HPI), which you want to add to the all list in the Reason for Visit list. Note: The reasons for visit are linked to an HPI template on the Manage Custom Links practice template. Select OK.

You can document up to six reasons for a patient's visit during intake. OpenNextGen® Enterprise EHR. Open theIntaketemplate, and then expand theReason for Visitpanel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window.
