Record Asthma History of Present Illness

You can document the Asthma - HPI details for a patient. The reason for visit added by the intake staff on the Intake template appears on the SOAP template. A value is filled in the Introduction field in the Reason for Visit panel. The introductory line displays any reason for visit selected during the current encounter. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: From the grid, select the asthma reason for visit. From the left pane, select asthma. The Asthma - HPI template opens. Document the Asthma Flowsheet

You can document the diagnosis and clinical assessment for a patient on the Asthma Flowsheet template. You can use the Asthma-HPI template to document the details on Asthma Flowsheet. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the SOAP template, and expand the Reason for Visit panel. The Asthma HPI template opens. Select Asthma Flowsheet. The Asthma Flowsheet template opens. Asthma Flowsheet To add a diagnosis, do the following: Select the Asthma Diagnosis field. The Diagnosis Search window opens and displays the asthma diagnoses. Enter Pre-Treatment Values for Spirometry

You can use the General panel on the Spirometry template to record the pre-treatment details for spirometry testing. Open NextGen® Enterprise EHR. In the Spirometry panel, select Add.The Spirometry template opens. Asthma Workflow

Asthma workflow is the process of documenting a patient's asthma related issues. The Asthma workflow enables testing or treatment of pulmonary function during a normal office visit. The possible workflow for asthma patients are: Intake staff uses the Intake and Histories templates to begin the visit for a patient. A provider reviews the intake information and adds additional details on the Asthma Flowsheet template. The provider uses the Spirometry and Nebulizer panels, respectively to document the pulmonary function testing and/or a nebulizer treatment of the patient. The provider uses the Finalize template to complete the visit and to calculate the E&M code. The front office staff uses the Checkout template to complete the discharge formality for the patient. Document Nebulizer Pre-Treatment Details

You can document a patient's nebulization values on the Nebulizer Treatment - Procedure template. You can edit or remove a nebulization treatment entry from the Nebulizer grid. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the SOAP template, and expand the Nebulizer panel. In the Nebulizer panel, select Add.The Nebulizer Treatment - Procedure template opens. Nebulizer Treatment - Procedure template In the Pre-Treatment panel, select the Description field and select an indication from the Proc Nebulizer category on the diagnosis search window. Note: You must record a diagnosis in the Description field to successfully bill the nebulizer treatments. NextGen® Enterprise EHR fills the Code fields automatically. Record Pulmonary Function Testing Results

You can use the Spirometry panel to document Spirometry results. The Spirometry panel is located on the Intake - Allergy and the SOAP - Allergy templates. Open NextGen® Enterprise EHR. Open the Intake - Allergy or SOAP - Allergy template. In the Spirometry panel, select Add.The Spirometry template opens. In the Pulmonary Function Testing panel, do the following: Enter Obstructive airflow limitation. Enter Bronchodilator responsiveness. Enter Ventilatory volumes and Lung volumes details. Select the Gas transfer level. Enter the Flow-Volume loop details. Enter the details in the Arterial blood gas results and Oxygen tension sections. Indicate the level of Carboxyhemoglobin and Acid-Base Interpretation Select the applicable checkboxes regarding plateau phase, COPD or asthma, and restrictive impairment. Select the applicable option for pre-operative quantitative perfusion scan. Select the quality of study. Select the date and comparison details if you have compared the current study detail
