Set Clinical Status or Resolved Date for Problems

You can set a clinical status or resolved date for a problem on the Problems panel on the Histories-Cardiology template. Open NextGen® Enterprise EHR. Open the Histories - Cardiology template. Select the Problems panel. In the Problems List section, select a problem to update the status.The details of the selected problem have been displayed in the fields. Problem List To update the clinical status or resolved date, do one of the following. To update the clinical status, click Clinical Status, select a status, and click OK. Update Existing Cardiac Problems

During a medical evaluation, if the details of a patient's documented cardiac problem need to be changed, you can modify the problem with the latest details in Problem List on the Histories - Cardiology template. When you modify a cardiac problem, if the SNOMED® code is changed, the existing problem record is updated as follows. In Cardiac Problem List, the problem record has been overwritten with new details. In Problem List, the old record is retained and a new problem record has been added. For example, if you modify a patient's Chest Pain Syndrome to ACS, there are two records; one for Chest Pain Syndrome and the other for ACS. Note: If you choose not to retain the old record in Problem List, you can either delete it from the Problems module or change the status of the old problem to resolved. You can update a cardiac problem in Problems List on the Histories template. Open NextGen® Enterprise EHR. Open the Histories- Cardiology template. Select the Problem List. Document Cardiac Problems

The Problems panel enables you to document, search for, and select the chronic and secondary conditions of a patient's problems. Open NextGen® Enterprise EHR. Open the Histories - Cardiology template. Select the Problems panel. In the Cardiac Problems List, click a link to open the template.The template associated with the link opens. Enter a date in the Date PopUp template and click Accept. In the Comments section, enter the history and details of the problem. Note: In case the patient has no problems, this field does not need to be added. Card CH Valvular Select Accept.The resulting window opens with all problems listed on the Problem List. Do one of the following on the problem list. Choose to add the current problem as a new problem. Merge it into an existing problem as a problem note. If you want to add a new problem, click Add new problem, and select the appropriate problem description from the resulting window. Add Cardiac Problems to Problem List

You can add a cardiac problem to the Problems List on the Histories- Cardiology template. Open NextGen® Enterprise EHR. Open the Histories- Cardiology template. Select the Problem List and click Add/Update.A diagnosis search window opens. Enter a problem in the diagnosis search window and click Search. Select a problem from the list and click Select.The Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code have been added and displayed in the Problem List panel. For all new problems, Mark as Chronic has been selected by default. Select clinical status, side, and site in Problem List. Select Onset Date and select the date of onset. Enter an approximate date if the exact onset date is unknown. To select the date when the problem was resolved, click Resolved Date. To mark the problem as a secondary condition, select Secondary Condition if the problem is not chronic. To add the selected problem to the tracked problem list, select Marked as Tracked Problem.

Patient's Problem list

You can do the following in the Problem List panel: Electronically record, modify, and retrieve a patient's clinical problem list to meet Quality Reporting requirements Document a patient's chronic conditions and ongoing clinical problems. View problems from past encounters Update problem details To indicate that the review of the patient's documented problem is complete, the provider must select the Reviewed check box.
