During a medical evaluation, if the details of a patient's documented cardiac problem need to be changed, you can modify the problem with the latest details in Problem List on the Histories - Cardiology template.
When you modify a cardiac problem, if the SNOMED® code is changed, the existing problem record is updated as follows.
- In Cardiac Problem List, the problem record has been overwritten with new details.
- In Problem List, the old record is retained and a new problem record has been added. For example, if you modify a patient's Chest Pain Syndrome to ACS, there are two records; one for Chest Pain Syndrome and the other for ACS.
Note: If you choose not to retain the old record in
Problem List, you can either delete it from the
Problems module or change the status of the old problem to resolved. For more information about deleting a problem, go to

- Open NextGen® Enterprise EHR.
- Open the Histories - Cardiology template.
- Select the Cardiac Problems List section.
- Select a problem record and click Edit.
A template corresponding to the problem opens.
- On the opened template, update the appropriate problem details.
Note: The History and Detail sections in the Comments section are available for editing, but not the History and Detail sections on top of the template.
- Select Update.
The problem on the Cardiac Problems List section has been updated.
