Edit an Existing Nursing Care Plan

The existing nursing care plan information can be updated when required. Open NextGen® Enterprise EHR. Access the Care Coordination Home Template. Expand the Nursing Care Plan History panel. From the Nursing Care Plan History panel, select the record that you want to update. Select Edit.The Nursing Care Plan Data template opens. Update the information on the Nursing Care Plan Data template following the instructions given for adding a nursing care plan. To save the updates, select Save & Close. Add a Nursing Care Plan Data

In the nursing care plan data, you can provide details of the identified problem, goal and outcome. Open NextGen® Enterprise EHR. Access the Care Coordination Home Template. From the Nursing Care Plan History panel, select Add.The Nursing Care Plan Data template opens. Nursing Care Plan Data In the following fields, type the appropriate text or select Common Phrases and select a phrase. Problems Goals Outcome Intervention Note: For the Problem, Goals and Outcome fields, you can also select Previous Problem, Previous Goals, or Previous Outcomes and select an entry from previously saved information. Select the Status field and select the current status. Provide the Start date, Next team review date and Next intervention due date. You can select a date from the displayed calendar. Nursing Care Plan History Panel for the Care Coordination Home Page

The Nursing Care Plan History panel lists all the nursing care plans created for a patient. Access the Care Coordination Home Template

Open NextGen® Enterprise EHR. On the Top Toolbar, select Templates icon. To view all the available templates, select All. On the Medical Records tab, search for and select Care Coordination Home. Generate Care Management Documents

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can generate the following documents from the Document panel: Nursing Care Plan Summary Transition of Care Comprehensive Assessment Return Visit Care Management Summary Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Document panel. To add additional records, fill in the fields as appropriate and select Add. To generate a nursing care plan Summary document, select the Generate Nursing Care Plan Summary link. To generate a transition of care document, select the Generate Transition of Care link. To generate a comprehensive assessment document, select the Generate Comp Assess link. To generate a return visit document, select the Generate Return Visit link. Document Care Coordination

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Care Coordination (BH), and then select OK. In the Time Entry Navigation sidebar, select Billing Time Entry, and then enter the start time. Note: You can select the current time or enter the time manually. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list.
