Patient Health Questionnaire (PHQ-2)

The Patient Health Questionnaire (PHQ-2) link is set up by the practice on the Practice Configuration template. The PHQ-2 is an optional setting and is a depression screening consisting of two questions. If the patient answers either of these two questions positively, a full depression screening is recommended, using the Patient Health Questionnaire (PHQ-9). If the PHQ-2 is positive, a link for the PHQ-9 automatically appears on the Depression Screening - PHQ-2 template. Parent topic: Patient's Review of Systems

Patient's Review of Systems

You can document the review findings for a patient after reviewing a list of systems. Review of Systems In Review of Systems, you can document symptoms for multiple systems on a single template with the use of one page ROS, or use the single systems in the left pane configured on the Framework Content system template. The patient health questionnaire PHQ-2 and PHQ-9 links appear on the section, based on the Show PHQ Link setting on the Practice Configuration practice template. Document Clinical Depression Screening

You can use the Patient Health Questionnaire (PHQ-2) button to document clinical depression screening and a follow-up plan. Open NextGen® Enterprise EHR. Open the Intake template. In the Review of Systems panel, select the PHQ-2 link.The Patient Health Questionnaire (PHQ-2) window opens. Select an option for each question. To open the Patient Health Questionnaire (PHQ-9) window, select the Patient Health Questionnaire (PHQ-9) link.The Patient Health Questionnaire PHQ-2 and PHQ-9 links appear in the Review of Systems panel based on the Show PHQ Link setting done on the Practice Configuration template. Document the Review of Systems Panel

The Review of Systems panel allows you to document the findings after reviewing a list of systems. Expand the Review of Systems panel. In the Review of Systems panel, select ROS from the left pane. Note: The ROS option is dependent on the specialty selected. For Ambulatory ASC specialty, you will see GI-ROS. The speciality specific review of system template opens. To document normal values in fields of all the sections, select the ROS defaults field and select the appropriate normal default set name from the list.All of the fields in the template appears with normal values to allow for charting by exception. If applicable, select the Neg or Pos option for the applicable systems in the template. You can select the system-specific link to add detailed findings on the system. If you want to save the documented values as the default set for the current template, select the Quick Save button and enter a default set name for the template.The default set name appears in the list generated by

Manage Pediatric Screening

You can document and manage the screening tests and results for a patient using the screening buttons in the Screening panel on the Pediatric SOAP template. Open NextGen® Enterprise EHR. Open the Pediatric SOAP template. To document the screening instrument, click the appropriate button from the Screening panel. To document the vision acuity details and the color vision test results, click Vision and enter the details on the Vision Screening - PE template. To document the patient's audiometry test details, click Hearing and enter the details on the Tone audiometry - PE template. To document the details of the tuberculosis risk assessment and skin test, click PPD and enter the details on the Tuberculin Skin Test template. Open Health Assessment Questionnaire Disability Index

Open the Health Assessment Questionnaire Disability Index from the Vital Signs [New Record] template, if necessary. Select HAQ-DI link that is available at the left side of the numeric keypad.The multi-page Health Assessment Questionnaire Disability Index template opens. Document the patient's ability for self-care on each page of the template, and select Next to go to the next page or Previous to return to the previous page of the template. Select Save & Close.
