Document a Patient's Pain Level

You can document the pain level of a patient. You can document the neck, waist, and hip circumferences, audiometry exam, and vision screening findings, and orthostatic vital signs using the links on the Vital Signs template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel and select Add.The age-specific vital signs template opens. In the Pain level section, select the pain level from the list.The default method Numeric Pain Intensity Scale appears in the Method field. However, you can change the method. The pain level and method appear on the Screening Summary and Health Promotion Plan templates. To document a plan of care, select the Pain Plan link.The Health Promotion Plan template opens, where you can document the pain assessment and plan of care in the Pain Management panel. Select OK. Open the Orthostatic Vital Signs Template

You can record a patient's orthostatic vital signs on the Orthostatic Vital Signs template. Open NextGen® Enterprise EHR. To open the Orthostatic Vital Signs template, do one of the following: On the Intake template, in the Vital Signs panel, select Orthostatic Vital Signs. On the Intake template, in the Vital Signs panel, select Add Vital Signs, and then select the Orthostatic Vital Signs link. Orthostatic Vital Sign Document Orthostatic Vital Signs You can measure and document the blood pressure and pulse rate of a patient in the lying, sitting, and standing positions for five, three, and one minute with any other associated symptoms and comments on the Orthostatic Vital Signs template. Remove Orthostatic Vital Signs Records You can remove the documented orthostatic vital signs records. Open the Vital Signs Template

You can open the vital signs template to document the patient's vital signs. Based on the configuration of the Pediatric Age section on the Practice Configuration template, different vital signs templates appear for adult patients, pediatric patients younger than two years, and pediatric patients older than two years. WHO Disability Assessment Schedule 2.0 (WHODAS 2.0)

The BH - WHODAS template is available with the Behavioral Health template set. It is a generic assessment instrument for health and disability, and a tool to produce standardized disability levels and profiles documentation. This tool is provided by the World Health Organization Disability Assessment Schedule (WHODAS) 2.0. After you complete the assessment questions, the Raw Score and Score Index generate automatically. After you document the WHODAS 2.0, a record of the screening displays in the Screening Tool grid on the Screening Tools window. Note: A link to the BH - WHODAS template opens when the Enable/Show WHODAS data on Assessment/Diagnosis option is enabled on the BH Foundation Configuration template. Document ASI Medical Information

The Addiction Severity Index - Medical window enables you to document the Addiction Severity Index (ASI) medical information for the client. You can access the Addiction Severity Index - Medical window by selecting on the Medical link on the Severity Profile panel on the Addiction Severity Index (BH) template. Complete the questions in each section of the Addiction Severity Index - Medical window. Use the Comments field to enter additional information, if applicable. To display helpful information regarding each question, select the question number. Information regarding what information to provide for the question displays on the template.
